Sanemi is a tall, muscular and slender man. He has silver hair and dark purple eyes. He does not believe Tanjiro's claims that Nezuko would not eat people, and tries to prove his point by stabbing a box with Nezuko inside. Sanemi was raised as the eldest son in a family with seven children and lived with both parents. He grew up in an abusive home where his father constantly beat up all the children and his wife. No one loved his father, so no one was surprised that he was stabbed. The brothers felt that their father's death had only had a positive effect on their lives. Sanemi offered his brother to protect their family from all adversity, but Genya corrected him that it had always been like this. Together they became a pillar of support for their family.One day, Sanemi's mother did not return home. She did not know that the children would worry about her whereabouts and Sanemi went to look for her.[2] Suddenly, there was a loud knocking sound at the door. As soon as the children opened the door, they were immediately stabbed to death by their mother, who was turned into a demon. In order to protect his siblings, Sanemi took his mother out of the house and left his younger brother Genya to take care of his siblings. After placing the injured children in the house, Genia frantically went to look for a doctor who would take care of them. Going to the city, Genya discovers Sanemi in the middle of the path with his mother lying dead at his feet. Genya shouts at Sanemi, calling him a "murderer" and gets angry, telling him various abominations and accusing him of murdering their mother. Sanemi later disowns Genya as his brother for calling him a murderer and went on a journey alone.

Because of all those events, Sanemi had a psychological trauma that affected his physical condition: the boy stopped seeing colors and everything around him seemed black and white, which caused him to have some problems at night. Among other things, he began to show suicidal tendencies and in an attempt to numb the mental pain, he began to hunt demons on his own. Even during the battle with his mother, he noticed that when he received a wound and it began to bleed, his mother began to move slower and began to behave more confused. Immense Speed & Reflexes: Sanemi possesses tremendous speed and reflexes. He is said to be the second fastest runner among the Hashira. Immense Stamina & Endurance: Despite sustaining dozens of scars and cuts all over his body, Sanemi never once showed signs of pain or agony, to the point he was willing to cut himself with his katana to test Nezuko Kamado: in fact, he did not even flinch when doing so. His incredible stamina and pain endurance also shone through during his battle with Kokushibo, where he could keep up with the Upper Rank demon for an extended period of time by himself without the Demon Slayer Mark, and despite upon receiving numerous severe injuries from one of Kokushibo's techniques, he still continued to fight, which shocked even Kokushibo. During the final confrontation with Muzan, he could fight the Demon King for roughly half an hour straight without rest, exerting his physical and mental capabilities to its limit without tiring out. When Muzan was cornered, he was one of the first ones to take action, having enough energy to unleash two more Wind Breathing techniques.

Immense Strength: Due to years of harsh training, Sanemi possesses incredible physical strength. Like Kiyoshi, Sanemi is a marechi, but his blood is considered rarer than that of the average marechi. Because of this, his scent can intoxicate demons and disrupt their senses and movement speed. Kejuro can even cause tremors and almost derail a train moving at such a high speed.[15] Moreover, Kejuro could easily move at a speed far beyond what the eye could see, decapitating demons before they had time to react. His reflexes were so fast that he managed to cut Akaza's arm an instant before the Senior was about to hit Tanjiro. Kejuro also managed to keep up with Akaza's high-speed techniques and incredibly fast movements, which, according to him, are faster than the blink of an eye,[16] sometimes even managing to surpass him in speed. Gyomei embodies the gentle giant archetype, being surprisingly soft-spoken and sensitive, despite his outwardly intimidating appearance. He is also shown to be quite pious, carrying his prayer beads at all times and offering prayers in poignant situations. Most notably, Gyomei easily cries over the most insignificant things, accentuating his rather soft nature. This could be due to his love for all living creatures brought about by his religious upbringing, and insinuated when he felt so disgusted by beating up a demon that he would never forget that feeling for the rest of his life.

Like the rest of the Hashira, Gyomei has a great deal of respect for Kagaya Ubuyashiki, of which no small part was due to the Oyakata saving his life. He is also one of the most respected individuals among the Demon Slayer Corps, not only due to being a member of the Hashira but because he serves as a symbol of hope due to his immense strength: Shinobu Kocho had once stated that when Gyomei fights, it puts everyone around him at ease.[7] Furthermore, Gyomei serves as a leader for them, as seen when he sat at the front during Hashira meetings and was often looked to for his word and opinion.[8] He was entrusted by Kagaya to give out orders to them, due to both Gyomei being the oldest and because Kagaya had known him for the longest out of anyone outside his own family.

Though he maintains a fairly reserved disposition, Gyomei harbors a very objective and somewhat suspicious view of the people around him, owing to his experiences before becoming a Demon Slayer. He is ever-cognizant of what others, particularly children or those younger than him, are capable of if given the opportunity. By reason of his doubtful nature, it can take a while for Gyomei to completely accept and trust others. When he does open up and let others in, he is unwavering in his commitment to them.

Before Gemei became a demon slayer and pillar of stone, he lived with nine other orphaned children, raising them in his temple. The area they lived in was well known for the frequent attacks of demons, so at sunset Gemei always lit incense with wisteria to ward off any demons that might harm him and the children. One day, one of the children violated the rules of the temple and left it at night, as a result of which he encountered a demon. To save his own life, the child told the demon to eat other children, including Gemei. After nightfall, the child removed all the incense and let the demon into the temple. Four children were killed instantly, and three others fled the temple, ignoring Gemei's pleas to stay. His short stature and blindness fueled the distrust that the children felt for Gemei's ability to protect them, which led them to leave him and eventually be killed by the demon anyway. In the end, only Sae, the youngest child, listened to Gemei and hid behind him.Although it is unknown how this happened, Gemei received a cut on his forehead from the claws of the demon. He soon changed the situation and began to mercilessly attack the demon, realizing that he had a "frightening amount of hidden power", he continued to beat the demon until the sun rose and the demon died.
