One of the most prominent features of Sanemi is his remarkable physique and the numerous scars that furrow his face, chest and arms which he does not hide and keeps as a reminder of how hard his job is and of which he is proud.

Regarding his attire, it consists of the Demon Slayer uniform, being in his case a customized version of his uniform which is black dyed green, having the upper part of it unbuttoned, exposing his torso and his multiple scars. He also has a white jacket, as well as keeps his Nichirin sword on his belt. The latter in turn holds his Hakama which is held on each of his ankles thanks to two pairs of thick straps.

Giyu's favorite thing to wear is a haori that has the right side solid red and the left side has a geometric pattern with squares of green, orange, and yellow.

Giyu displays an emotionless appearance with a stoic and carefree expression on his face. He is very reserved and has a strong sense of justice. He is decisive when it comes to killing other Demons and has no respect or mercy towards them like most Demon Slayers. Giyu has a complex character and cares too much about what others might think of him. He is not very talkative and finds it extremely difficult to relate to others, so he usually keeps his distance. His inferiority complex is so strong that he even considers abandoning his position as Water Hashira. Despite his distant and stoic personality, he also has a very protective and caring side with his friends.

Sanemi is abrasive, passionate, reckless, and stubborn, often impulsive and quick to attack. He is usually indifferent towards others and can get angry very easily. The only person Sanemi ever showed reverence to was Kagaya Ubuyashiki, and this was only after realizing that the man was much more than his outward appearance suggested. Sanemi has also shown a hunger for battle, wishing he could meet the Demons that are part of the Twelve Demon Moons.

Sanemi's cold, rude, and disdainful behavior is a result of losing too many people he cared about in the past. Later, when he sees those people who have passed away, such as his mother, he can't help but give her a smile so he can see her again.
