Kokushibo was a Demon who maintained his human features despite having devoured countless people for 400 years. He maintained a series of quite unusual characteristics: he was a man of tall stature, pale skin tone, and an athletic, muscular build; His hair was long and black with burgundy edges, held by a long ponytail that reached almost to the height of his lower back.

On his face, he possessed a series of flame-like marks that covered the left area of ​​his head and the right side of his neck; These marks were, in his time as a human, his Hunter Mark. Kokushibo's most emblematic feature and, in turn, one of his most sinister hallmarks were his three pairs of eyebrowless eyes, which had a yellowish color to their irises and a red sclera divided into segments. In the eyes of the center, the numbering of him is located as the Superior Moon One identified with the kanji of First crescent indicating his dominant position within the Superior Moons.

As for his clothing, he wore a kimono with purple and black patterns with a white haneri (undergarment of a Kimono), a black hakama, and a white obi belt. He possessed a katana made from his own flesh and bones, which was completely decorated with eyes on the cutting blade, on the Tsuba and the Tsuka. The guard was also covered with flesh and eyes to completely increase the degree of his peripheral vision.
