Akutagawa has a slim build and very pale skin. He often covers his face with his hand because of coughing. He has short, black hair. Two longer strands frame his face on the sides. Closer to the tips, they change their color from black to dirty white. The bangs are short and almost completely open the forehead. Akutagawa has deep-set, narrow eyes. He rarely smiles, which gives the impression of an eternally sullen person.

He is wearing a long shirt with three layers to the bottom, which he wears over narrowed black trousers. Each layer has a wide cut, which creates a "valana" effect. He wears a white frill over the neck of his shirt. The sleeves of the shirt are lace and visible from under the sleeves of a long black coat with a high, standing collar, which Akutagawa wears over it. After Higuchi and the Lizards rescue him, he apologizes to her for the problems he caused. Although these are small, brief moments of Akutagawa's calmer and more serious side, they nevertheless mean a lot. Obviously, Akutagawa rarely or never lies, which makes him honest, even if in most cases it is cruel and inappropriate.
