Sentomaru is a 33-year-old man who is 2.79 meters tall. Sentomaru is a tall, large man with a robust build similar to that of a sumo wrestler. His face and haircut, however, resemble those of a happy child, with a large scar that runs from his left eye to the left corner of his mouth.

His outfit consists of a crimson red shirt that resembles a giant bib, featuring an emblem on the front that means "sky," and a large red and black rope called a Tsuna. Sentomaru also has bandages wrapped around his right fist and a kanji symbol meaning "battle" tattooed on his left bicep.

After two years, he is shown wearing a Navy coat draped over his shoulders, signifying his acceptance into the Marine Corps. He has also removed his bandages.

Sentomaru is seen as an apathetic and stern individual who proclaims himself the man with the tightest and toughest defense. Sentomaru also claims to have the tightest mouth and refuses to answer any questions. However, despite this assertion, he is willing to share information related to the question. Whenever he gives away information, he states that he is doing so freely and not at the person's request.

Vegapunk saved a young Sentomaru, which led to his loyalty as a way of repaying this debt. Vegapunk mentions this as a means of pressuring Sentomaru to convince him to challenge the World Government so that he can protect him.
