Yuji is a muscular teenager with large light brown eyes and spiky black and pink hair. After eating the finger and consequently becoming a host for Sukuna, he receives a second pair of eyes that only open when Sukuna controls the body. After the events in Shibuya, Itadori gained a large scar between his eyes.

Yuji wore a light-colored hooded jacket, black high-hemmed pants and white shoes. In the anime, however, the color of his pants was denim and he wore red shoes.

Yuji is a fair person who cares a lot about not only his comrades, but anyone he cares about. see them as people with their own wants, despite how deep or superficial your connection to them is. On Halloween, Geto starts his plan by applying a large screen around Shibuya. Civilians are arrested and panic ensues, just as the villain predicted. The choice for Halloween was not a mere coincidence — on a date like this there are more people on the street. Geto began turning civilians into curses with Mahito's help, doing this to attract the attention of the jujutsu sorcerers and manage to seal Gojo in a curse. Several people died and the city of Shibuya was completely destroyed. Nobara was 'killed' by Mahito, as Mahito touched her face and almost killed her in front of Itadori. Megumi was almost killed by a bounty killer named Toji, but Megumi managed to survive, barely. After Nanami was partially torn apart by Dagon's domain, he was burned by Diogo, Diogo is a bounty killer, before dying at Mahito's hand, Mahito blows up half of Nanami's body. Mahito killed and blew up Nanami in front of Itadori. Gojo, Itadori's teacher, was sealed in a Geto curse. Gojo is still alive, but was taken by Geto and Mahito.
