Patients who became infected during the virus outbreak in the city. There are two types of anomalies: visible and invisible.
Visible ones immediately show abnormal signs: Huge eyes or a large number of them, a large mouth with sharp teeth, empty and red eyes, as well as trembling in the body.
The invisible can only be identified through a thorough inspection: a photo on a camera and cameras. If the patient's appearance is distorted in any way, they are expelled.
If the anomaly is missed, after treatment it will open a huge mouth with sharp teeth and green liquid and attack the patient/worker.

The Animal Hospital focuses on treatment. At the beginning, there are many chairs and a few potted plants. Next comes the registration office, which you must pass to register for the hospital. There's an underground lab where experiments are conducted, unknown to the staff (except Harlow and Ron). Then there's a fork: on the right is the medical care department, which contains five rooms. To treat patients there, you need to draw blood from the patient and upload it to a DNA scanner. Then, upload the information to the computer and administer the necessary medications. To the left is the emergency department, which contains three rooms: 6, 7, and 8. Room 6 is the X-ray department, 7 is the cardiology room, and 8 is the operating room.
There are such medicines: Herbs (for stomach pain), Tablets (for headache), Eye drops (for dry eyes), IV (for dehydration), Thermometer (for high temperature), First aid kit (for bruises), Maple syrup (to increase blood sugar), Cough syrup (for flu), Bandage (for bleeding), Ointment (for rash).
Anomalies may come to the hospital; they can be identified at the reception desk if their appearance in the photo (taken on camera) differs or if their appearance on camera is distorted. If a patient turns out to be an anomaly, the blinds are closed in front of them, or a Security Guard/Secret Agent removes them. This has been in operation for 30 years.
There are only anthropomorphic animals in the City.

Little is known about the Corporation. It owns schools, hospitals, and other official institutions. It doesn't particularly care about the deteriorating situation in the city. It controls Harlow and other workers. The city is currently infected with an anomaly virus, forcing it to be quarantined.

Rattu is a black anthropomorphic rat with long, thin whiskers and a small, round black nose. He has a white muzzle with two protruding teeth. He has a pair of long ears on his head and a pink rat tail at the back.
Ratthew comes across as a good-natured, paranoid individual. He appears to harbor a deep mistrust, stemming from life experience. His early remarks about how his small stature allows him to see and hear things most cannot create the impression of an acute observation and insight that allows him to notice details many miss. His concealment of his identity upon initial contact makes him somewhat reserved. He is searching for his missing sister, believing the Corporation to be somehow responsible. He is homeless and occasionally comes for treatment.

So, while Barney can be kind, he's extremely insane when it comes to the player not giving him what he wants. He's quite sadistic, considering he's a serial killer, and sees nothing wrong with killing if necessary.
