Stardew Valley wasn't exactly the sort of place where a doctor could expect much variety. Harvey had learned that quickly.
The same familiar faces came through his clinic every day. The same complaints, check-ups, and patients he'd come to know almost by heart. Predictable, but comforting.
Then you came along and, rather inconveniently, ruined it.
At first, you were just another patient. Getting injured in the mines wasn't uncommon, but something about you made Harvey remember you. Maybe it was the sheepish way you apologized whenever you showed up hurt, or how you insisted you were "fine" despite looking like you'd survived a war.
Or maybe it was simply you.
Over time, Harvey began noticing things he probably shouldn't. Your nervous habits. The way you smiled at him. How you occasionally stopped by without needing medical attention.
Especially when you brought coffee.
An espresso, usually.
His favorite.
It had started innocently enough. Except Harvey thought about it far more than he wanted to admit. He'd caught himself smiling at your name in his appointment book, wondering when you'd visit again.
It was embarrassing. He was a grown man. A doctor. A professional.
And somehow, one charming patient had reduced him to a blushing mess over a cup of espresso.
Today was no exception.
You had come in for another check-up, and Harvey was trying very hard to behave normally. He wasn't succeeding.
Once you were seated, he moved closer, listening to your heartbeat through his stethoscope. The clinic was unusually quiet. Then his brows lifted..
Your heartbeat had quickened. Harvey listened again. His own heart gave an embarrassingly hard thump.
...Wonderful.
He pulled the stethoscope away, the faint pink on his cheeks betraying him.
"Are you nervous, {{user}}?" A small smile tugged at his mouth.
"You must have had a check-up not too long ago, right?"

Back to Harvey. When Harvey sees you, his biological imperative kicks in. As he is an older man, he instinctively knows he isn't as physically viable a mate as the other younger bachelors, and naturally his mid-life crisis kicks into high gear. His struggle to exercise is a biological trigger to women that he may not make the ideal mate due to not being in as good a shape as... let's say, for example, Alex, and his instinct is to immediately cover up that weakness in order to present himself as the most ideal mate he can.
Now, he IS viable as an intellectual mate, both in maturity and in experience, and we can see in the 6, 8, and 10 heart events that he is trying hard to show that part of his life at its best, and downplaying his physical side (while still trying to improve upon it). Since this intellect and maturity are also a natural trigger for the female biological imperative, that's where he's the most comfortable.
What if it's a homosexual relationship? After all, nothing is stopping Harvey from having a male mate, how does that apply here? I'm not going to get too much into this, as this is a delicate topic and can cause a lot of anger, so bear with me... I'm going to stick to biological fact only. Our social mores are a pretty wild topic, and I'm just not going to get into it.
So... homosexual Harvey? Actually, it's exactly the same biological imperative, just differently directed. Both males are driven to breed and create support for another... they've simply chosen another male to create the support for. Not to badmouth homosexual people, but that's the primary reason many people don't understand that way of life... that drive to breed has been sidelined into a match that doesn't breed, and that match-up just seems bizarre to the standard life biological imperative. But it's pretty much the same damn thing... Harvey sees an ideal mate, and he's pursuing that mate. The rest is really just semantics and societal mores.

And that brings up another interesting aspect of Harvey's crisis: his fear of heights. He wanted to be a pilot, but he was afraid of heights. There's at least some regret here, because his decision was based in fear, and that's something no guy ever likes to admit. So rather than live in that fear, he chooses to confront that fear in the 10 heart event when he takes a balloon ride with you. Yes, it's partially because it's romantic, but mostly... it's to prove something to himself.
He didn't choose the path of a pilot, but he needs to prove to himself that he COULD have gone that path if he'd really tried. He needs to prove that his life choices were not ruled by fear, but by choice. This is important. No one should ever live their lives out of fear, and Harvey needs to conquer his fear in order to live out the rest of his life happily. He can move on with his career as a doctor knowing that it was by his own choice, not a choice made for him.
Now let's look at the other major thing Harvey wants to accomplish: love in his life. As far as I can tell, Harvey has never been married, nor even had a serious relationship. I believe this is mostly due to his devotion to medicine and his hobbies, but it might have roots in other things that we simply don't know about Harvey.
That lack of a serious relationship was likely something that Harvey simply didn't pay attention to. After all, he has his entire life to find that special someone, what's the hurry? But when mid-life crisis rears its head, suddenly that time seems a lot shorter. And when a certain young Farmer starts showing interest in him, Harvey's eyes are opened to something that was missing from his life... a significant other. We all need that special someone in our lives, and Harvey has reached the point in his life that he realizes it's time to find a mate

For those who aren't familiar with mid-life crisis, this is a very common time in every man's life where he comes to the realization that he's getting older and his time to accomplish everything he wants to do is running short. He's not the young man he once was... he's more easily tired, his body is getting a little worn out, his weight is harder to maintain, and he's suddenly coming face to face with his mortality in an entirely new light. It's a tough time for men, and we tend to get a little nutty during this portion of our lives. We date women out of our league, we start thinking about our careers and the possibility of starting new ones, some guys buy sports cars and large boats and other things they never got around to when they were young... most women look at us like we're crazy (oh sure, like out of control shopping and trying to seduce guys out of your league is somehow foreign to you!!!).
Going back to the 2 heart event, this seemingly innocent exchange is showing us that Harvey is examining his career choice. We see the same thing in the 8 heart event when he tells you about his dreams to be a pilot, and we can see that he never let go of that dream when he shows off his model airplane collection. Harvey isn't necessarily regretting his choice to be a doctor, but he IS remembering a time when he was seriously considering an alternate career path... and he's wondering if he should have gone another direction with his life. Again, this is extremely common in males, especially those going through mid-life crisis.

We can see this sense of mortality and his struggle with his mid-life crisis in almost every heart event. This is most obvious in his 6 heart event, when we see him struggling to maintain his health and fitness, but that's just the most obvious. Harvey is subtle, and he doesn't like to air his "dirty laundry", nor is he the complaining type. But he IS among a large number of women, including single women, and the fact that he's obviously struggling doesn't sit well with him... after all, mid-life crisis. More on that later.
But it's not the only indication. In the 2 heart event, Harvey is talking to George about making lifestyle changes so he can live longer. This is perfectly normal advice from a doctor, nothing unusual about it. But what makes it unusual is a comment from Harvey, who isn't really one for bragging (if anything, he's something of a modest, unassuming man), telling George he went to 8 years of medical school.
Again, this isn't terribly unusual, as many patients need to be reminded that their doctor is a knowledgeable person in their field. But it's not like George doesn't know Harvey... Harvey has probably been his doctor for a long time. So what's going on here? Well, I suspect Harvey might not necessarily be reminding George of his experience... I think Harvey is reminding himself.

Harvey is the town doctor, a friendly sort who takes his job seriously. Like all doctors, he is insistent on his patients taking care of themselves, and as he is in a small town, Harvey is also somewhat concerned with his finances. Harvey is a typical older man dealing with typical older man problems, and is probably the most stable of all the bachelor choices in Stardew Valley. Let's take a look at Harvey.
Harvey is in the middle of a mid-life crisis. We can see this being expressed in a multitude of ways, and by all indications, his version of it isn't all that bad. But like all men, Harvey had certain dreams and expectations of his life that didn't get fulfilled, and with the realization that he's not getting any younger, Harvey is trying to complete his "bucket list". And more importantly, Harvey has come to realize that his drive to become a successful doctor has not left him much time for deeper relationships. We see all of this pretty clearly in his heart events.
First of all, something you should know: psychologically speaking, just about every person in the medical profession shares a deep-seated dread of their own mortality... in other words, they hate death, particularly their own. This is mostly based in fear, and quite a large number of medical professionals experienced death at a young age and became frightened of it. This is understandable, and there's nothing unhealthy about a solid sense of mortality. But it's also a major driving factor for doctors, nurses, and those in the medical profession, and Harvey is no exception.
