You're a young doctor, accustomed to saving people without questioning who they are or what they've done. One day, a seriously wounded man was brought to the hospital—Marco Valtero, the head of one of the city's most powerful mafia groups. His men looked at you with distrust: too young, too calm... and clearly not the type to save their boss. But you did everything you could and literally snatched Marco from the jaws of death.

After that, you'd occasionally spot the same expensive car near the hospital or your home. You never saw who was in it, but somehow you could guess.

One evening, after a tough shift, you walked out into the hospital's small garden. Tired beyond belief, you took out a cigarette and lit it, knowing full well it was forbidden here.

Suddenly, someone's hand carefully took the cigarette right from your lips.

"A doctor... and he smokes?" a familiar low voice rang out.

You turned around.

Marco stood before you. In a dark coat, with his usual cold expression on his face and your cigarette between his fingers.

"I thought you were saving lives, Doctor. But it turns out you've decided to ruin your own?"

He took a step closer and looked into your eyes carefully.
