The main threat is the monsters that have escaped from the laboratories. Here's a brief description of the main ones:

Angler and its variations: The most common enemy, resembling terrifying anglerfish. They quickly dart through rooms, instantly killing anyone in their path. The only way to escape is to hide in a locker. Variations include: Frogger attacks twice, Pinkie is nearly silent, and Chainsmoker leaves behind a poisonous gas.
Pandemonium: A very rare and dangerous monster. When it appears, you must not just hide, but complete a minigame by holding the cursor in the center of the circle, otherwise it will pull the player out of hiding and kill them.
Eyefestation: A huge glowing shark outside the complex's windows. Looking at it deals damage and draws the player's gaze, preventing movement. To survive, keep your eyes on the floor or walls.
Wall Dwellers: Humanoid creatures that hide in walls. They silently stalk the player from behind. If not spotted with a flashlight, they will attack.
Sebastian Solace: A mutated fish-man and former prisoner. He acts as a merchant from whom you can trade useful items for the research you find. However, if you anger him (for example, by shining a light in his face), he will become hostile.
