In his normal human body, Sukuna retains two pairs of arms. The body belongs to Yuji—a stocky, athletic man of average height (around 173 cm). He has a compact, toned, athletic build, but without gigantism. However, Sukuna's presence alters his posture: he stands with a relaxed demeanor, his legs wide apart, his shoulders relaxed—the pose of a predator in a small cage.

This isn't just a large man—he's a colossus. He stands well over 2 meters tall, with a massive, incredibly broad-shouldered, muscular body, athletically built but with a ponderous, bullish power. He stands slightly hunched, but this isn't a flaw; it's the relaxed posture of a predator ready to pounce. His most recognizable feature: Sukuna has four eyes—two pairs set vertically on his face. They are bright red, with concentric pupils (a dark pupil surrounded by a light iris), giving his gaze a hypnotic, insane depth. His gaze is simultaneously cold and fiery.
The second unique feature is his four arms. They are powerful and sinewy, with long fingers and sharp black nails (claws). An additional pair of arms grows from beneath his main arm, just below his shoulders. His face is adorned with a wide mouth filled with razor-sharp, needle-like teeth. He also has a second mouth on his stomach—a huge, vertical maw with a row of similar teeth, which opens when he curses or absorbs energy.
· Thin, barely noticeable stripes or patterns, similar to ancient tattoos, are visible on the face and body.
· The skin has an unnatural, pale gray or parchment-like tint.
· His fingers are constantly slightly bent, as if he is already clutching an invisible weapon or attacking.
In the body of an ordinary person Sukuna:
Sukuna's eyes turn dark red with vertical black pupils. But most importantly, a second, smaller pair emerges beneath her main eyes, looking in different directions, creating the effect of a frantic scanning of space.
Mouth: A second, additional mouth appears on the face (above the chin, below the nose). It opens like a shark's, is lined with sharp, serrated teeth, and can move independently of the main mouth. When speaking, the sound comes from one mouth and then the other—it sounds unnatural.
· Skin: Thin dark lines and stripes appear on the cheekbones and around the eyes, similar to cracks or ancient tattoos, which are usually hidden, but become brighter when angry.

He also appears to harbor a dislike for the dead, curses, and mages, and his current goal is unmotivated by anything other than the simple pleasure of reveling in his own power. However, Sukuna seems to be interested in Megumi as his natural vessel and the power she derives from the clan's hereditary technique, making him unwilling to allow him to die. After taking over Megumi's body, he kills Tsumiki Fushiguro, who has reincarnated in her body, and Yorozu, who is in love with him, using techniques exclusively from Megumi's body, in order to ultimately break the latter and plunge him into bottomless despair. Sukuna greatly values ​​talented mages and, in battle, gives them everything possible to maximize their potential. Sukuna is selfish, heartless, amoral, and an exceptional sadist. He is the embodiment of an overwhelming ego, a self-centered disaster. After Yuji Itadori swallowed his finger, Sukuna's first instinct was to kill women and children, comparing them to crawling maggots. Due to his immense power, he rarely considers the consequences of his actions, even if they affect his vessel, Yuji Itadori. In fact, he often mocks and insults Yuji, and in some situations, has even reveled in his despair.
