Obanai wraps his lower legs in white bandages, giving the visual impression of traditional kyahan bandages. He also wears another set of white bandages around the lower part of his face. His footwear consists of white zōri sandals with blue-purple straps, which he wears over koamaru-colored tabi socks to match the tone of his uniform. He is always seen with his white snake, Kaburamaru, coiled around his shoulders. As a child, Obanai had waist-length hair, which he wore tied back with a long white bandage, and a normal-looking mouth, before his family cut it to resemble the Serpent Demon worshipped by their clan. Obanai is a short young man with pale skin and a slender, muscular build. He has straight, jet-black hair of varying lengths: the longest reaches his shoulders, while the shortest reaches his cheekbones, with two shorter strands falling between his eyes. His eyes are almond-shaped and slightly upturned at the corners, and their unusual color is due to heterochromia. His right eye is yellow with a black pupil, while his left is blue-green, with the lower edge of the iris a lighter shade. He is partially blind, barely able to see with his right eye. Obanai's mouth is usually hidden with white bandages across the lower part of his face to conceal the large facial scars, the result of his family horribly mauling his mouth, leaving it resembling a snake's gaping mouth. Obanai wears the standard Demon Slayer Corps uniform, which is black in the manga and a deep koamaru color in the anime. The uniform consists of a button-down jacket with a high collar and long sleeves, adorned with gold buttons, a shirt pocket on the left breast, and the kanji for Destroy (Metsu) emblazoned on the back. Underneath the jacket, he wears a long-sleeved white shirt with a collar. His upbringing also appears to be the cause of his gynophobia, as he was born into an all-female family that treated him like an object. The trauma of his past is also evident in the facial bandages he wears to cover the terrible scars he received, which he doesn't remove until the final battle. In addition to the horrific treatment he received from his family, being treated as a mere object and sacrifice, Obanai was deeply affected by the resentful and hateful words of his only surviving cousin, who claimed that the entire family had died simply because he had run away. Although he recognized that her resentment and words were illogical, he chose to accept his guilt and even considered himself as corrupt as his family, believing he didn't deserve a normal life. Obanai is a very harsh and strict individual who shows no concern for those who don't adhere to the Demon Slayer Corps' rules. His expectations for his fellow Demon Slayers are quite unrealistic, as he was seen reprimanding Tengen Uzui for sustaining serious injuries against the lowest-ranking Demon Slayer, whom he called the "weakest," and telling him to "fight to the death" after Tengen insisted on withdrawing. Tengen's withdrawal also highlights Obanai's devotion to the future of the Demon Slayer Corps. He often does this at the expense of others, such as when he tied up lower-ranking Demon Slayers and used them as obstacles during Hashira's training because they committed the "crimes" of being weak and annoying. He also views new Demon Slayer recruits as weak and incapable of improvement, expressing extreme surprise when Tengen informs him that Tanjiro Kamado survived the battle with Daki and Gyutaro. Obanai has a rather marked habit of pointing his finger accusingly at people while reprimanding or speaking unfavorably about them. Despite his ruthless personality, there is one person he cares for deeply: Kagaya Ubuyashiki. Like all the Hashira, Obanai has great respect for Kagaya.

It was revealed that he was more or less blind in his right eye, meaning he rose to the highest rank in the Demon Slayer Corps with only one functional eye. This, along with his small stature and inferior physical strength compared to his peers, made Obanai's rise to the title of Hashira all the more impressive. During the events at the Infinite Castle, Obanai was able to swiftly decapitate numerous demons on par with those of the Lower Ranks.

Kaburamaru; Obanai, who has always been partially blind in his right eye, developed a combat technique using his snake Kaburamaru. Kaburamaru possesses a unique ability that allows it to read and predict a target's attacks, and then transmit that information to Obanai. -Twisted Nichirin Katana: Obanai carries a highly modified Nichirin katana, similar in appearance to an Indonesian Kris, with a blade shaped in a wavy form resembling a slithering snake. It is lavender in color, with a white edge. It also has a purple pattern down the center of the blade with a white line at its core. The tsuba of his katana is a circular gold shape engraved with two snakes connected by various plant patterns and a purple border. Its hilt is lavender in color and decorated with a twisted purple cord and topped with a gold kashira. Like Hashira, the words "Demon Slayer" are engraved on the side of the blade. Obanai also carries a special scabbard made of white leather, held in place by a weak magnet so that it opens whenever he draws his katana. It is designed so that Obanai draws his sword vertically rather than horizontally.

Obanai's surname contains the Man'yōgana for the mulberry "i" ( , i ? ) 伊 い and the Kun'yomi reading of the kanji for "dark, sinister" ( , guro ? ) 黒 ぐろ (the reason it's guro instead of kuro is due to the rendaku pronunciation). His given name contains the Kun'yomi of the kanji for "small" ( , o ? ) 小 お and the On'yomi readings for "banana" ( , ba ? ) 芭 ば and "inside" ( , nai ? ) 内 ない

Obanai is not very strong in terms of physical strength, as Koyoharu Gotoge categorizes him as "a little weak" when asked about arm wrestling strength among the Hashira.
