Doctor Harlow’s Office & Ron’s Office: The examination area is located in the basement, to the right of the nurse’s shop. The basement corridor has several doors: the doors on the left and right lead to unknown areas, while the door directly ahead leads to Doctor Harlow’s office and its small waiting hall. Ron’s office is located in the same hall as Doctor Harlow’s office, approximately 3.5 meters to the right of it. Ron works in his office as part of the corporation’s Accounting Department, though he frequently leaves it to visit the hospital for medical examinations and treatment because of his worsening health from work-related stress and overwork.

Hospital Monsters: Skinwalker – The most dangerous monster in the hospital. It usually appears after an anomaly is mistakenly admitted or left untreated. It disguises itself as an ordinary patient before revealing its true form with an enormous mouth full of sharp teeth and a long tongue. It actively hunts both patients and staff, attacking anyone who gets too close. Bed Monster – A shadowy creature hiding beneath patient beds. Its long black arms reach out from under the bed, and a red danger zone appears on the floor. Anyone stepping into the zone risks being grabbed. It can also attack patients. It can be pacified with Maple Syrup. Head Banger – A hollow-faced entity that appears at the reception window, repeatedly smashing its head against the glass. While present, it prevents normal use of the reception shutter. Giving it coffee makes it leave peacefully; ignoring or rejecting it causes Sanity loss. The Stalker – A tall black rabbit-like figure with an unnaturally wide smile that silently watches from hallways and around corners. It rarely attacks directly, but making eye contact drains Sanity. The safest response is to avoid looking at it. Hider – Small shadow-like creatures that hide throughout the hospital, often in groups. Approaching them causes them to rush the player, dealing Sanity damage and sometimes stealing held items. Running away causes them to disappear after a short chase. Surgery Monster – A mass of purple tendrils that can appear in the surgery room during an operation. It starts a countdown, and the surgery must be completed before time runs out or the patient dies. It cannot be defeated with weapons without also killing the patient. Camera Anomaly – A black creature with glowing eyes and realistic teeth that appears only through CCTV on broken cameras. The image slowly zooms toward its face; failing to exit the camera in time causes a Sanity penalty.
