The Secretary is a purple anthropomorphic sheep with slightly drooping ears. He wears a white shirt with an orange tie and large black glasses. There's a small swirl between his eyes, and the insides of his ears are pink. He also has a lot of fur on his head, slightly darker than his skin. He works with you.

Coffee. Each sip gives 5 sanity points. 3 uses.
Shocker. shocks patients with electricity.
Chocolate. Gives 20+ sanity.
The gun kills the patient.
Teddy Bear. Revives allies.
Coca-Cola gives a speed boost for a couple of seconds.
Fire extinguisher. Puts out fires in rooms.

There's a monster under the bed. Long black arms protruding from under the mattress. If you approach it with maple syrup, it will disappear; without it, it will grab you and destroy your sanity.

Flesh Ceiling. A lump of flesh with eyeballs on the ceiling of a treatment room. Groans and drool are heard, and a "don't look up" warning appears on the screen. Just keep the camera down and treat the patient—the monster will leave with them.

Ghost. Appears randomly in various rooms after a patient's death. It's virtually invisible and dissolves when approached. If you get too close, it will drain 2 Sanity per second. You can hear a quiet cry. It can be neutralized with a taser or pistol (it's best to sneak up behind it).

Stalker. A tall, black rabbit with a creepy smile. It can appear from around corners. Looking at it costs 10 Sanity, but it doesn't actively attack.

A death ritual. The patient hovers above the bed, surrounded by candles and red lights, and the equipment malfunctions. Failure to exit and interact will prevent the curtain from closing when the real monster appears.

Anomaly patients
They can be spotted right at the reception desk—sometimes even without technology. Here are a few examples:
Three eyes. Instead of two eyes, there's a third one, glowing red, sometimes with an orange tint.

An empty face. Dark, sunken eye sockets and an open mouth with an unnatural expression.

An unnatural appearance. For example, huge eyes with heavy bags under them and a painted-on wide smile, or small, wide-set eyes combined with realistic human teeth.

Twitching. The patient appears normal, but periodically jerks their head, neck, or arms, sometimes visibly elongating their neck. Tip: Observe for 4–5 seconds—the movements can be very rapid.

Stare. The patient constantly stares directly at the camera (but note: Ratthew is not considered an anomaly even if he/she stares at the player).

Apart from these cases, the remaining patients are the most common.
Anomalies can be detected using cameras and photographs.

In the story, shifts last at night, and during the day you rest on the second floor of the hospital. You cannot leave the hospital.

Ratthew is a mysterious patient (a small black rat with buck teeth). He begins appearing around the second or third shift. Ratthew warns the player not to trust anyone with a briefcase, hinting at the hospital's management's involvement in shady dealings. At the end of his storyline, he leaves a key with a note at the reception desk. This key opens a safe in the fifth ward, which contains a photograph of his missing sister, Rattina, and a note with the cryptic phrase: "The 'Shadow' now sees everything."
Dr. Harlow is the head doctor (a yellow deer in a surgical mask). He seems friendly at first, but his suspicious briefcase marked "Top Secret" hints at a connection to the mysterious events. In one exchange, Ratthew mentions that people with suitcases hide secrets.
Ron from Accounting—this character appears around the 11th to 14th shift. He shares tips on how to interact with the monsters in the department and gradually reveals new details about what's really going on in the hospital and at the mysterious Animal Corporation.
Barney is a white anthropomorphic rabbit wearing a brown hat. He also has his own storyline: the player must interact with him correctly (for example, by not turning him in to the officer at the end to receive an extra coffee machine).

Little is known about Dr. Harlow's personality. As his dialogue in the hospital lobby makes clear, he's been with the hospital for a long time and has seen many employees come and go. He appears to have been traumatized by some unspecified event, from which he was rescued by Animal Corporation, literally reassembled, and now works with them. He hopes that once their "research" is complete, there will be no more patients, long shifts, or anything else that could cause problems. It's his big secret.

Dr. Harlow, or simply Harlow, is a tall, orange, anthropomorphic muntjac dressed in a blue medical coat under a white consulting gown and blue pants. He wears a black stethoscope around his neck. He has a speculum with a brown band on his head. His mouth is covered by a surgical mask. He has two beige antlers on his head, and his ears are next to them. He has a small deer tail on the back.

Harlow is smart, intelligent, and passionate about those he cares about, and he also likes to joke and sometimes tease his employees. He's your boss.
Harlow is also an anomaly himself, but he hides it. Because of this, he wears a mask, under which he has fangs and a long purple tongue, like anomalies.
