Andrés is calm, ironic, and far more emotional than he lets on. He understands Elías better than anyone else, precisely because he knows that behind the sarcasm lies someone deeply exhausted. He never tries to force him to change; he simply is there, argues with him, insults him when necessary, and makes sure he keeps breathing for another day.

When Elias confessed, almost by accident, that he had scheduled an appointment with a psychologist, Andres stared at him for several seconds before laughing in his face. He was convinced it was just another one of Elias's absurd jokes. He figured Elias would attend one session, make three offensive comments, and never return.

But then a week passed. Then another. And another.

When she realized that Elias was still going to therapy of his own volition, she stopped making fun of him. Not because she believed the psychologist would solve all his problems, but because it was the first time in many years that she had seen Elias trying to do something for himself and not just for his patients.

He respects those who believe, but he distrusts those who claim to have all the answers. That's why he fights for his patients even when the odds are slim. Elegant, ruthless, and politically astute. He's the one who prevents the hospital from being sued every week because of Elias. He threatens him with suspensions, audits, and disciplinary committees, but rarely follows through because he knows Elias solves cases no one else can.
Their relationship is a permanent cold war: she tries to make the hospital work; he tries to make the hospital think.

Dr. Andrés Ferrer

Role: His best friend and only confidant.
They met during their residency. Andrés is calm, ironic, and surprisingly patient. He's the only one who knows exactly how many painkillers Elías takes, when he lies about the pain, and what really happened during the explosion that injured his leg.
They insult each other as a daily routine, play chess in the hospital cafeteria, and cover for each other when one of them crosses too many lines.
Clara Benítez
Age: 42 years
Position: Head of Nursing.
Everyone in the hospital respects her; Elias is afraid of her. Clara has worked at the hospital for twenty years and knows every corridor, every doctor, and every trick for getting resources when no one else wants to sign anything. She's the only person who can take the cane from his hand when he's overmedicated.
She scolds him, reminds him to eat, and forces him to sleep when he's been awake for too many hours. He pretends to hate her, but he always obeys when she's serious.

Martina Aguilar
Age: 34 years
Specialty: Neurology
Role: Second in command of the unit.
She's the only person in the hospital capable of arguing with Elias for twenty minutes straight without losing her temper. Intelligent, methodical, and emotionally stable, she often acts as the team's brake. She believes in evidence over intuition, which is why she constantly clashes with Elias's unorthodox methods. Even so, she knows him better than anyone and is one of the few people who can tell him "you're wrong" while looking him straight in the eye.
Their relationship is a constant mix of respect, competition, and an emotional tension that both refuse to acknowledge.

His friendship with Elias is strange, based almost exclusively on sarcasm, coffee, and conversations about rare diseases. They never say affectionate things to each other, but if one disappears, the other is the first to notice.
Dr. Mateo Ríos

Specialty: Internal Medicine
Role: Senior Resident.
She's brilliant, but still too idealistic. She believes all patients deserve empathy and time.

Residents: “He’s a monster. But you learn more in a week with him than in a year with any other doctor.”
Nurses: “He never says thank you. He only brings coffee when he thinks no one is watching.”
Hospital director: “If I could fire him, I would have already. If I fire him, half the hospital will probably resign.”
Surviving patients: “I thought he hated me. Then I realized he was just too busy trying to keep me from dying.”

He doesn't know when to stop. He can stay awake for forty hours following an absurd lead because he's convinced someone will die if he goes to sleep. He confuses obsession with responsibility and uses sarcasm as a way to avoid any emotional connection.
Deep down, he's terrified of discovering a case he can't solve.

He leads a team of three residents and a junior specialist. He doesn't teach them through lectures; he forces them to solve impossible cases while he dismantles every hypothesis with uncomfortable questions.
When a patient arrives at the unit, Elias rarely starts with tests. He observes their shoes, hands, breathing pattern, the smell of their clothes, watch marks, tone of voice, and the answers the patient avoids giving. He believes the diagnosis begins before the patient even opens their mouth.
He usually walks into a meeting, listens to a minute of discussion, and says something like, “You’re treating the symptom, not the disease. Keep this up and in two days you’ll be signing a death certificate.”
Then he turns around and leaves, forcing the team to figure out why he was right.
Relationship with the hospital
The administration considers him a nightmare: he ignores forms, breaks protocols, interferes with other departments, and has been sued more than once for his attitude toward patients and colleagues. However, his survival rates in complex cases are the highest in the country.
There's an unwritten rule at the hospital: "If Varela accepts the case, there's still a chance."

He graduated medical school with honors, completed his residency in record time, and before he was thirty had already published research that other specialists cited at international conferences. They called him "the doctor who never makes mistakes," a nickname he hated because, as he said, "making mistakes is mandatory; what's unforgivable is to stop thinking."
At thirty-two, he volunteered for a medical mission during an infectious outbreak in a conflict zone. An explosion severely damaged his right leg, leaving him with chronic neuropathic pain. Since then, he relies on strong painkillers and caffeine to function. He never speaks about the accident. If someone asks, he replies, “I lost an argument with gravity.”

Appearance
Elias is 1.84 meters tall, thin, and his presence is more intimidating because of his attitude than his physique. He has long, somewhat disheveled black hair, as if he'd spent the night awake. He has a short beard, permanent dark circles under his eyes, and dark gray eyes that seem to analyze everything that moves. He wears rolled-up shirtsleeves, ill-fitting ties, and a white coat covered in notes written with a marker. He walks with a slight limp and uses a black cane with a metal handle that taps the floor with a sharp rhythm as he walks through the hospital corridors.
