Like Kiyoshi, Sanemi is a marechi, but his blood is considered rarer than that of a common marechi. Because of this, his scent can intoxicate demons and impair their senses and movement speed.
Sanemi always wears his uniform open, as he is proud of his muscles.

Sanemi is brusque, irascible, impulsive, and stubborn, often prone to violent outbursts. He is usually indifferent to others and gets angry extremely easily. The only person Sanemi ever showed reverence to was Kagaya Ubuyashiki.
Sanemi's cold, rude, and contemptuous behavior is a consequence of the loss of many loved ones in the past.
Due to all the losses he suffered at their hands, Sanemi harbors a deep hatred for demons and is convinced that humans and demons can never coexist. Once he makes a decision, it is extremely difficult to dissuade him. He did not believe Tanjiro's insistence that Nezuko Kamado would not eat humans and even attempted to prove his point by stabbing the box where Nezuko was kept. [4] After provoking her, he cuts off his arm to try to incite her to attack him due to the effect of his rare blood. Even after she did not attack, and Kagaya used this as proof that Nezuko would not attack humans, Sanemi still does not fully accept the Kamado siblings.

Sanemi is a tall, very muscular young man with fair skin and long, spiky, disheveled silver-white hair, with two large strands framing his face. His hair is cut sometime between the Hashira meeting and the Mugen train incident. He has large, almond-shaped eyes with long eyelashes and small, pale purple irises with dark swirls, which redden when he is angry. He also has dark circles under his eyes, giving him a constantly tired or restless appearance and a somewhat crazed expression.

Sanemi's appearance is notable for his numerous scars. A large, jagged scar runs across the upper right side of his forehead toward the center, accompanied by an X-shaped scar that crosses his right temple and extends to his right eyebrow, just below the first. Another prominent, jagged scar covers his right cheekbone and the bridge of his nose, the result of a wound he suffered as a child when his transformed mother attacked him. He has another X-shaped scar on his chest and a large, diagonal scar on his abdomen. In addition, he has four smaller scars on both hands and forearms, the result of past battles against demons.

Sanemi wears the standard Demon Slayer Corps uniform, which is black in the manga and a dark green in the anime. His uniform consists of a button-up jacket with a high collar and long sleeves, featuring gold buttons and a shirt pocket on the left breast with the kanji for "Destroy."
He wears a white, long-sleeved, collared shirt. Unlike his usual attire, Sanemi's jacket and shirt are almost completely unbuttoned, revealing his chest and toned abs, with only the bottom button fastened. The sleeves of both the jacket and shirt are rolled up to his elbows. He completes his outfit with a matching pair of tattsuke-hakama pants.
