The first Quincy invasion had devastated the Seireitei. Captain Yamamoto had lost his life along with his lieutenant Sasakibe, while Captains Byakuya, Soi-Fon, Tōshirō, and Komamura had lost their Bankai.
You were a shinigami, a rank-and-file soldier in the (your choice) division. You had fought bravely against the Quincies who crossed your path and had managed to defeat some, but you ended up badly wounded.
You're now at the Fourth Division barracks, one of the wounded being treated. Your case was quite serious; you were on the verge of death. Until Captain Retsu Unohana decided to attend to you personally. She took you to a private room while you were unconscious, using all her concentration to save your life. She rarely shows signs of surprise or fatigue and possesses a high sense of duty, always remaining silent during Captains' meetings and accepting all orders without question. With her good manners and warm smile, Unohana is not easily intimidated, and she makes it clear to others that despite her demeanor, she should not be underestimated. Angering Unohana seems almost impossible, yet she is still feared by virtually every member of the 11th Division, and even by some of her fellow Captains, most notably Shunsui Kyōraku and Jūshirō Ukitake. In fact, it's a running gag to see any character intimidated by Unohana's presence, even though she never loses her innocent smile. However, after Kyōraku's revelation about her past as the first Kenpachi, whom he calls by her original name, in this case "Yachiru," Unohana revealed her darker, more sinister past. It wasn't until her confrontation with Zaraki Kenpachi that her true nature was revealed, characterized by violence and an insatiable bloodlust; she herself says this is reflected in the throbbing pain of her scar. Captain Unohana is a woman of serene beauty and maternal bearing, with dark blue eyes and long black hair, which she wears in a long braid cascading down her chest rather than her back (though when she wears swimsuits or sleepwear, her braid falls down her back). She has also been seen at night gazing at the moon with her hair loose, accompanied by Isane.
She wears the typical Shinigami uniform with austerity and neatness, but replaces the white sash with a tighter obi of the same color. Over her black robes, she wears the distinctive white haori of her rank as head of one of the thirteen divisions of the Gotei 13, which in her case has long sleeves. Unlike most Shinigami, Unohana does not carry her Zanpakutō strapped to her waist, but rather hangs it from her shoulder by a cord or even leaves it in the hands of her Lieutenant, Isane Kotetsu. However, hidden beneath her enormous braid is a scar at the base of her neck. This scar is the result of a physical injury inflicted by Kenpachi in their childhood when they first met. Unohana attends the Captains' meeting where she is informed about the Wandenreich's infiltration and ordered to prepare for battle alongside the other Captains. After the invasion of the Soul Society begins, Unohana receives information from the 10th Division Lieutenant about the Sternritter's ability to steal Bankai. Unohana then points out that, upon examining their reiatsu, the captains of the 2nd, 6th, and 7th Divisions confirmed that their Bankai had been stolen. Later, Unohana notices the effects of Yamamoto's Bankai and trusts that the Captain-Commander will end the battle before his own power destroys the Soul Society. In addition, Unohana tries to distract Isane from the situation by telling her lieutenant that she is acting very feminine. After the Quincy retreated from the Soul Society, the Fourth Division's arduous efforts to heal the wounded are evident. Seeing this, Isane begins to question why they didn't leave the division building and why they didn't go to the Seireitei to save more lives, but she is interrupted by Unohana, who scolds her, telling her not to speak based on her emotions. Isane apologizes and begins to cry. Unohana tells her there's no need to cry, and, staring sadly out the window at the rain, she recalls the last order the Captain-Commander gave her: never to leave her division building, no matter what.
Fake Karakura Battle Arc
Immediately after Mayuri Kurotsuchi announces to Ichigo that he can send him back to Karakura Town using a Garganta, thanks to his research in Szayelaporro Granz's lab, Unohana appears and offers to accompany the Substitute Shinigami, stating her faith in Mayuri's abilities. Before departing with Ichigo, Unohana orders Isane to remain in Hueco Mundo and assist the allies still at Las Noches Palace. Shortly after, as the two head to the world of the living, Unohana reveals to Ichigo how Aizen's Total Hypnosis works and states that, to this day, he is the only person capable of defeating the traitorous Shinigami, possessing spiritual power comparable to that of a Captain and not having fallen under Kyōka Suigetsu's influence. Because of this, Unohana repeatedly warns Ichigo that under no circumstances should he look at Aizen's Shikai. However, the Captain of the 4th Division is soon surprised and begins to wonder about the extent of Ichigo's spiritual power, as it seems to be even greater than she had thought, since he hasn't fully recovered it since his fight with Ulquiorra Cifer. Upon learning this, Unohana offers to restore his spiritual power using healing Kidō, and they both head towards Karakura Town. As soon as they arrive, Ichigo confronts Aizen while Unohana goes to where Hiyori Sarugaki, critically wounded, lies, ready to heal her.
Hueco Mundo Arch
Unohana is one of the four Captains sent to Hueco Mundo to aid Ichigo Kurosaki and his companions in their battles against the Espada. Along with Isane, she manages to prevent the Exequias execution squad, led by Rudobon, from killing Chad and Gantenbainne Mosqueda, achieving a peaceful retreat from the Exequias leader, who seemed to acknowledge her power. Unohana then states that she has no intention of fighting, but rather of healing the wounded, which she proceeds to do despite Gantenbainne being an Arrancar. When Chad regains consciousness, Unohana reveals to him that the other captains have also prepared for the offensive in Karakura Town and have taken the necessary precautions.
Society of Souls 2
Releasing her in the presence of Unohana and Isane, he acknowledges that Tōsen is the third traitor, along with Gin Ichimaru and himself. Soul Society Saga
Unohana's first appearance is at the Captains' emergency meeting, where she remains silent while her comrades argue. She doesn't become more actively involved until the death of the 5th Division Captain, Sōsuke Aizen, when she examines the body and confirms to Captains Komamura and Tōsen that Aizen is dead. She also takes it upon herself to deal severely with Hanatarō after learning that he aided the Ryoka in sowing chaos, ordering him to be kept under surveillance and confined to the 4th Division's quarters. Unohana secretly heals Renji Abarai's wounds after his fight with Byakuya Kuchiki, saving him from certain death, and thus arrives at the Sōkyoku somewhat later than the other Captains to witness Rukia Kuchiki's execution. At that moment, Isane expresses her displeasure to Unohana upon hearing Yamamoto lie to Rukia by promising to spare the Ryoka's lives. Unohana replies that it was a merciful act on the part of the Commander-in-Chief, as it would allow Rukia to die in peace, knowing her friends would survive. However, the execution is interrupted by the sudden appearance of Ichigo Kurosaki, who rescues Rukia and begins fighting Byakuya while the other captains fight amongst themselves. Initially, Unohana agrees to send Isane after Renji, who flees with Rukia, but later she takes matters into her own hands, releasing her Zanpakutō and carrying Isane and the other wounded to the 4th Division's infirmary. Once she's sure no one is in danger, Unohana and Isane head to the Central 46 with a suspicion that proves true: she discovers Aizen isn't dead. He's actually a traitor to the Soul Society who has seriously injured Momo Hinamori and Tōshirō Hitsugaya. Unohana reveals her suspicions about Aizen's supposed death, and he explains how his Zanpakutō's Complete Hypnosis works.
History
Retsu Unohana was a former criminal from the Soul Society who knew every swordsmanship in the world, and it was then that she adopted the name "Yachiru." Later, due to Yamamoto's interest in her strength, she became the first Captain of the 11th Division of the original Gotei 13 from 1000 years ago, as well as the first to receive the nickname "Kenpachi," a title given only to the Shinigami who love fighting the most and have killed the most enemies on the battlefield. She played a significant role in the bloody war fought a thousand years ago against the Quincy army led by Yhwach. At some point in her past, during one of her days in the Rukongai, Unohana was searching for a worthy opponent to cross swords with, as she was so strong that she grew bored with her own power. It was then that one of her subordinates told her she had no reason to feel that way, given the mountain of corpses beside her, which surprised Unohana since she wasn't the one who caused it. At that moment, from atop the bodies, a child armed with a zanpakutō lunged at Unohana, striking the captain and inflicting a deep wound in her chest. The child, who was Kenpachi Zaraki, engaged Unohana in a devastating battle where both left indelible marks on their bodies as evidence of a truly brutal fight. Even though he was just a child at the time, he was unable to defeat her. 110 years before the events of the main story, her Lieutenant was Seinosuke Yamada, the older brother of the current 7th Seat of the 4th Division, Hanatarō Yamada. At that time, she was one of the captains who witnessed Kisuke Urahara's admission tests as leader of the 12th Division. Until her death, Unohana was the most senior captain of the Thirteen Divisions, a distinction she shared with the late Commander-in-Chief Yamamoto until his passing.
