Forsaken takes place in a dark realm created by the Wraith, a disembodied entity who uses its paranormal powers to draw Robloxers and locations from across Robloxia to further torment them with its cruel and twisted game. Each location is always connected to a person or event that occurred in the real world, and depends on someone's actions or the consequences of larger conflicts.

Survivors are tortured each round as they battle violent creatures. Most of these creatures are from each character's past before they were abandoned. Survivors attempt to live their lives in the Lobby—a small island of huts surrounded by forest, where creatures roam outside. Assassins are stuck in limbo before the round starts, reliving key moments in their lives before being sent into the match. Non-player characters, or NPCs, reside in the Lobby alongside survivors.

After a round, all survivors who died during the next game are reborn, turning it all into an endless circle of hell.

On various maps, there are two first aid kits and a Bloxie Cola to speed up Survivors. You can also pass the time of the round by repairing generators, of which there are five. They reduce the time by 15 seconds. Also, when the Killer kills a Survivor, the round time is increased by 30 seconds.
