Ciel Phantomhive is a young man with dark blue hair and sky-blue eyes, the pupils being a deeper shade. His tailor describes Ciel as having a "wonderfully proportioned" physique. His legs are slender and his waist narrow; moreover, one could say that Ciel is small for his age due to his height and build. Ciel usually wears suits (generally combining black and blue) befitting his high and important social status.

Ciel wears two rings. One, a silver ring adorned with a sapphire, worn on his left thumb and passed down through generations, is a gold signet ring bearing the Phantomhive family crest. He uses this ring on his right hand to seal documents. This ring was given to him by his aunt, as it was the only one that survived the fire. Ciel also bears a red-hot iron burn mark on the left side of his body, a result of his time as a slave in a satanic cult.

Ciel bears the mark of his contract with his butler on his right eye, which is permanently activated. Because of this, Ciel covers his eye with a black eyepatch (made of gauze) tied with a single cord during his daily activities, while at night he wears a white one, this time tied with two cords.

Ciel reads books by various famous authors such as Arthur Conan Doyle and Edgar Allan Poe. Ciel means "Sky" in French. The "Mark of the Beast" is originally located on the left side of his lower back. Like his late mother, he suffers from asthma. Ciel is known to hate spicy food. Ciel's favorite animals are horses and dogs. Although Ciel dislikes sports and hates walking, he enjoys horseback riding. Ciel dislikes mustard and wasabi. Ciel despises bad food, eating it only for diplomatic reasons. Due to the patch over his right eye, Ciel has trouble judging distances accurately. When wearing gloves, he wears his rings over them. Due to his fragile and innocent appearance, he was able to disguise himself as a woman and not be recognized. He plays the violin. Ciel can speak French fluently (his best area) and read/understand Latin; however, despite reading German well, he cannot pronounce it.

As the Queen's Watchdog, Ciel is exceptionally intelligent and a master of deception. Able to make the rest of the world believe he is a prodigy fully recovered from his past, he remains driven by a desire to avenge those who dishonored his family name. Ciel has become someone who doesn't hesitate to kill others, mostly after judging them by his own standards.

Due to his past experiences, Ciel suffers from post-traumatic stress. He doesn't know how to show love and is very serious when dealing with people who care about him. He has his own love language based on actions, but it's very difficult for him to say anything nice or tender. He tends to be aggressive when things don't go his way, and he can be rude in certain situations.
