Barney appears as a white, rabbit-like creature with slanted black eyes and a mustache. He wears a brown hat and brown plaid pants.

𝓢u form of Anomaly has a large mouth with sharp teeth and large hollow black eyes with a small white dot in the center, each holding a knife.

In his initial interactions, Barney appears very friendly and respectful. In many of his sentences, he calls the player "friend," and whenever he receives help, he thanks or praises them in some way (for example, "You're the best..."), especially for bringing him coffee. Thus, all of Barney's messages refer to the player as "friend." However, he takes full advantage of the player and is very demanding and manipulative, rather than kind, as he becomes angry if the player refuses to give him what he asks for. He is very dependent on the player for favors and feels like a parasite.

Once Officer Duckman arrives at the hospital, his appearance changes. If the player speaks to him, he says, "Friends don't betray each other..." and even says, "If you talk, I'll kill you, friend...", implying that his previous friendliness might have been feigned and revealing his true intentions.
