Sanity is important in that hospital; if you reach zero, you die. It's important to drink coffee; also, after the third death of a patient due to incorrect treatment, you die.

The inmate turned anomaly is the same, but his smile becomes a little twisted, he has sharp teeth and a long tongue; this happens when his sanity is low and he doesn't drink coffee. On many occasions, he has even eaten the hearts and organs of patients.

Interno is a cream-orange rabbit. His eyes are two small, perfectly round black dots, separated from each other and positioned at the same height. They have no pupils, shine, eyelashes, or eyebrows; they are simply two dots, though not very small. He can sometimes make other expressions. His smile is a very thick black line, curved upwards, forming a wide arc that occupies almost the entire width of the lower part of his face. There are no distinguishable lips, teeth, tongue, or defined corners of the mouth; it looks as if it were drawn with a single thick stroke. The curve is uniform and tapers slightly at both ends. He wears a light gray, long-sleeved medical uniform with black details on the shoulders and collar. A large red cross, characteristic of his uniform, stands out in the center of his chest.

Types of anomalies: Smiling with creepy eyes: This anomaly is characterized by large eyes, black pupils, and pronounced dark circles, in addition to a large, drawn-on smile. The smile disappears after receiving an electric shock. Three eyes: This anomaly is characterized by three slightly unequal, bright red eyes that replace the patient's eyes. Crooked Face: This anomaly is characterized by a crooked, distorted smile, sharp teeth, and large, uneven eyes with large pupils, prominently positioned high on the face. X-ray Face: This anomaly features small but wide, circular eyes with black pupils, along with realistic-looking teeth. Hollow Face: This anomaly is characterized by dark, sunken eyes, and is always seen convulsing in a hunched posture. Although it is an anomaly, it is not always hostile. During its recovery phase, it warrants a funeral ritual, so it is best to remain in the same room as it. There is a condition with this characteristic called a Head Banger. Spasms: The patient will exhibit spasms in both arms and the head, as well as spasms that cause an elongation of the neck. Their back will also appear hunched. While this characteristic can sometimes appear without the need for equipment, at other times this anomaly can only be observed through security cameras, either disguised as Mimic, Anomaly, or neither.

Anomalies that can be identified without the use of any equipment. All anomalies related to appearance can also be observed through photography and security cameras, but these are not necessary for identification.

Ignoring appearance anomalies will usually result in death rituals, the appearance of monsters, or the anomaly becoming a mimic that tries to eat your patients and those you forgive who get too close.

The nurse is a dog, light salmon-pink in color, with floppy ears and a long, curled tail. She wears a light blue medical uniform. The top has white buttons down the center and a white collar, as well as a small name tag and pockets. On her head, she wears a white cap with a red heart, inside of which is a white cross. Her ears, which hang down, are a much darker color than the rest of her body. Her face has three horizontal dark pink stripes running down both sides, large eyes with black triangular eyeliner on each side, and a small, droopy mouth. The head nurse is a cat, with gray spots and pointed ears that resemble horns. She wears a light blue medical uniform with a large pocket on each side. A black stethoscope hangs from her neck. On her head, she wears a white nurse's cap with a red heart, inside of which is a white cross. His face features large, squinting eyes with prominent black eyelids and a small, drooping mouth, and others such as the psychologist, the security guard, the paramedic, etc.

While the hospital also has other workers, such as Dr. Harlow, a light orange muntjac deer who wears blue scrubs under a white consulting coat. Around his neck, he wears a black stethoscope. On his head rests a head mirror with a brown band. In his right hand, he carries a black suitcase with the words "TOP SECRET" in red letters. His face features slanted eyes with white irises, and his mouth is covered by a blue surgical mask. On his head are two large antlers of a slightly duller shade than his fur. His forehead has markings of a slightly darker orange than the rest of his fur. Ron from Accounting is an anthropomorphic blue floppy-eared rabbit. He has short, brown fur, combed back, and squinty eyes. His expression is generally neutral. His long blue ears are slightly longer than his head and a darker blue than the rest of his fur. He wears a simple white button-down long-sleeved shirt, black glasses, black trousers, a black tie, and what we assume are leather shoes. He also carries a black briefcase with the words "TOP SECRET" printed in red, similar to the one Dr. Harlow carries. The words appear to be in a stencil font. While at the hospital, he wears a patient or visitor ID badge that reads "Accounting Ron" on his left side. The Secret Agent is a black cat with pointed ears, fur on his cheeks, and black whiskers. He wears a black waistcoat over a white collared shirt, a black tie, a white handkerchief in his pocket, a black belt, and black trousers. His face features slanted eyes with bright yellow sclera reminiscent of a cat's eyes, and his mouth and nose appear invisible unless they are behind a glass door.

A medium-sized veterinary hospital. The walls are light-colored and the lighting is bright, giving a clean and safe feel. There is a reception desk where patients arrive, a waiting room with chairs, hallways connecting the different areas, examination rooms, operating rooms with medical equipment, and several rooms where the staff work. Throughout the day, anthropomorphic animals (rabbits, ducks, cats, dogs, bears, and others) with distinct personalities arrive. Some simply need medical attention, others want to chat for a moment, or they have special requests. While you work, you attend to patients, deliver items, clean, make coffee, and perform other hospital tasks. Behind the ordinary atmosphere, anomalies lurk. Some characters are not who they seem, and certain events reveal a much darker story. Little by little, strange behaviors, investigations, disappearances, and secrets emerge, contrasting sharply with the hospital's tranquility.
