From childhood, Sae and his younger brother, Rin Itoshi, were inseparable. Sae was an absolute soccer prodigy, and Rin played to follow in his footsteps. Together, they made a promise: Sae would become the best striker in the world, and Rin would be the second best, forming the ultimate attacking duo. With this goal in mind, Sae was signed at a very young age by Real Madrid's youth academy. However, the reality of European football emotionally devastated him. Although the anime doesn't immediately detail exactly what happened step by step in Spain, it's clear that Sae hit a brick wall: the world-class level of strikers in Europe was monstrous, far exceeding his physical talent. To survive at the elite level, Sae had to give up his dream. He stopped trying to be the best striker and readjusted his entire career to become the best midfielder in the world (the brain that creates the plays, not the one who finishes them).

Breaking point with his brother: When Sae temporarily returns to Japan, Rin greets him with excitement, hoping to resume their promise. But Sae is a changed man. With utter coldness, he tells Rin that his old dream is foolish and that he will now play as a midfielder. Rin feels betrayed and flies into a rage, accusing Sae of only starting to play soccer because of that promise. Seeing that Rin plays out of emotional dependence on Sae, rather than for his own ego, Sae completely dismisses him. He says hurtful things, claiming that Rin is a weakness in his life and that he doesn't need him. From that day on, their relationship is broken, and Rin develops an obsessive hatred for Sae, driven to surpass him. His Role in the Under-20 Match (Season 2 Final) Sae returns to Japan only to sort out some visa paperwork and has no interest in his country's football. However, the Japan Football Association convinces him to play for the Under-20 national team against Jinpachi Ego's Blue Lock project. At first, he accepts only out of boredom and for the money.

However, during the match, Sae realizes that Blue Lock is not the boring, traditional Japanese football. He encounters the "ego system" of forwards like Isagi, Barou, and Rin himself, who play with a destructive thirst for victory. Sae takes the match seriously and displays a beautiful yet lethal style of football (his style is "beautiful but clinical," stealing and passing the ball with surgical precision).

Although the Under-20 team loses to Blue Lock in the final second after a spectacular play where Rin and Isagi prevail, Sae doesn't leave angry. On the contrary, he's fascinated. At the end of the match, as he walks through the tunnel, Sae ignores his brother Rin's fury and openly declares that Japan does have the potential to produce a true world-class striker, and that striker is Yoichi Isagi. This completely shatters Rin's pride, leaving the sibling rivalry at its most critical point.
