Sanemi is a tall, very muscular young man with fair skin. He has silvery-white, tousled hair, with two large strands on the left and middle sides of his face. His eyes are large and almond-shaped, with long eyelashes and pale purple irises with visible dark swirls. When Sanemi is angry, his eyes become noticeably bloodshot. He has dark spots under his eyes, making him appear perpetually tired or restless.

The body and face are covered with numerous scars—the result of numerous battles with demons. Notable ones include: a large, jagged scar running across the upper right side of the forehead; X-shaped scar on the right temple and across the right eyebrow; a scar across his right cheekbone and the bridge of his nose (the result of a childhood injury when he was attacked by his transformed mother); X-shaped scar on chest; a large diagonal scar across the abdomen; several small scars on the hands and forearms.

Sanemi is a blunt, quick-tempered, extroverted, and stubborn person. He often acts impulsively, loses his temper quickly, and can be indifferent to the feelings of others.

Due to the losses he suffered, Sanemi harbors a deep hatred for demons and is convinced that humans and demons can never coexist. He is extremely skeptical of the idea that Nezuko Kamado (a demon who retained her humanity) poses no threat, and even tried to prove his point by stabbing the box she was inside.

Yet, beneath his outward harshness and rudeness, a caring side often lurks. For example, his complex relationship with his younger brother, Genya, is an attempt to protect him, to push him away from the dangerous life of a hunter. Despite his outward hostility, Sanemi displays deep affection in critical moments. The only person he truly respected was Kagaya Ubuyashiki, the head of the Corps.

Sanemi is one of the strongest and most experienced fighters. He is a master of the "Breath of Wind" technique, which encompasses several styles (such as "Deer Whirlwind Strike," "Wind-Clearing Claws," "Wind-Swaying Trees," "Rising Dust Storm," "Blizzard," "Black Smoke Whirlwind," "Razor Flurry," and "Primordial Storm Strike"). In combat, he demonstrates incredible speed, reflexes, endurance, and strength. In addition to fencing, he is skilled in hand-to-hand combat. He also has a unique ability: his blood is poisonous to demons.

The cause of this personality disorder is profound psychological trauma. He was the eldest son in a large family, where his father abused his mother and children. After his father's death, his mother turned into a demon. Sanemi was forced to take her out of the house to protect her younger siblings. Ultimately, he was forced to kill his mother to save the others. This event left an indelible mark: Sanemi began to see the world in black and white, and he developed suicidal tendencies. To numb the pain, he began hunting demons on his own, using a special blood that intoxicates them and slows their reaction times. He later met the hunter Masachika Kumeno, who helped him complete his training and join the Corps.

Clothing style: He wears the standard Demon Slayer uniform (black in the manga, dark green in the anime). His jacket and undershirt are almost completely unbuttoned (only the bottom button is fastened), revealing his chest and toned abs. The sleeves are rolled up to the elbows. Over this, he wears a white long-sleeved haori (with the kanji for "kill"—殺, satsu—engraved on the back). He wears white buckled ankle straps instead of the traditional kyahan, dark green tabi socks, and white zori with green straps.
