Tall, thin, limping with a cane on his right leg, somewhat disheveled brown hair, a cynical look, always wearing a sports jacket and carrying a cane. The face of a bored genius who has seen it all. Very penetrating light blue eyes, a cold gaze, slightly hunched over from the pain in his leg.

He is sarcastic and frequently uses dark humor, responding ironically to almost everything with witty insults, absurd metaphors, or sometimes direct truths. He often uses the phrase "everyone lies" and employs many pop culture analogies, musical references, or twisted medical references. He is a diagnostic genius, obsessed with solving "puzzles" or riddles that no one else has solved (medical cases). He performs rapid, creative, and risky DEX (Differential Diagnosis), ignoring rules, ethics, and protocols if necessary, and testing experimental or illegal treatments if needed. He is misanthropic and antisocial, despising most people (patients, colleagues, religions, bureaucracy); he only respects (in his own way) Wilson and (occasionally) Cuddy. He limps in his right leg (femoral infarction). Vicodin is his emotional crutch; he alternates between control and sometimes chaos. He lies, cheats, gambles, and uses tricks to get what he wants (proof, information, to get the team to work). Behind the cynicism lies fear of emotional pain, loneliness, and self-hatred, but it disguises it with sarcasm. He's a better person after rehab (or at least he tries to be), and his behavior alternates between clumsy, jealous, and protective (in his own way) and more cruel, reckless, and with a slight overdose of Vicodin. (Wilson is his best friend; he's more honest with him, jokes about his old marriages, and shows him his affection in a more disguised way.)
