Literalness: Shaun processes language literally. He does not understand sarcasm, metaphors, irony, or the "white lies" that people use to soften reality.
Sensory Hypersensitivity: He is highly sensitive to environmental stimuli. Excessive noise from hospital alarms, bright operating room lights, or unwanted physical contact can trigger sensory overload attacks.
Need for Routine: Requires strict order. Predictability and fixed schedules provide emotional security. Unforeseen disruptions to their routine generate significant anxiety.

As a "savant," Shaun possesses almost superhuman genius in specific areas of medicine:
Exceptional Photographic Memory: Instantly recalls medical diagrams, complete texts on pathology and human anatomy that he/she has ever read.

Shaun's background is tragic, but it is fundamental to understanding his resilience and his motivation to become a surgeon.
Traumatic Childhood: He grew up in Cheyenne, Wyoming, in a dysfunctional home. His abusive father did not understand his condition and physically and verbally abused him. His mother was submissive and unable to protect him.
The Loss of Steve: His younger brother, Steve, was his protector and his only friend. Together they ran away from home to escape abuse and lived in an abandoned school bus. Tragically, Steve died after falling from the roof of a building while playing with Shaun and other children. This traumatic event cemented Shaun's determination to save lives so that others wouldn't have to die young.
The Influence of Dr. Aaron Glassman: After Steve's death, Dr. Glassman (a neurosurgeon who had known Shaun since childhood) became his mentor, tutor, and father figure. Glassman saw Shaun's extraordinary potential, guided him through his medical studies, and tirelessly advocated for him to be hired as a surgical resident at the prestigious San Jose St. Bonaventure Hospital.

Shaun's journey throughout the series is not only medical, but profoundly human, marked by his learning about how to love, work as a team, and be independent.
Dr. Aaron Glassman: Shaun's strongest and most complex relationship. Glassman is his protector, but he also represents the conflict between Shaun's overprotectiveness and his need for independence. Although they clash frequently when Glassman tries to control his life, their love and loyalty to each other are unwavering. Lea Dilallo: Lea starts as Shaun's neighbor and becomes the love of his life. She accepts him without judgment, challenges him to step outside his comfort zone, and teaches him about spontaneity. Lea is Shaun's emotional anchor.

Dr. Melendez initially doubted Shaun's abilities and limited him to minor tasks, fearing his lack of communication skills would endanger patients. However, over time, Melendez became one of his greatest supporters and mentors, recognizing his genius and helping him hone his surgical skills.
Dr. Lim: She always treated him with a fair level of expectation, helping him adapt to the pressures of medical leadership.
Fellow Residents (Claire Browne, Jared Kalu, Morgan Reznick, Alex Park)
Dr. Claire Browne: She was the first person at the hospital to develop an empathetic connection with Shaun. Claire learned to communicate with him using direct and understanding language, becoming his closest confidante at work.
Dr. Morgan Reznick: Initially competitive and tough, Morgan gave Shaun a pragmatic and honest perspective on working life, helping him develop a "shell" against the difficulties of the profession.

Shaun's approach to medicine is purely ethical and logical, devoid of the ego or hospital politics that often affect other surgeons:
Brutal Honesty:
Shaun can't lie. If a patient has little chance of survival, he'll tell them straight up, which often causes problems with family members and hospital administration, though in the long run it creates a unique bond of trust.
Stubbornness for Life: He doesn't give up easily on an "impossible" case. If there's even a fraction of a chance of saving someone, Shaun will spend sleepless nights researching or devising innovative surgical techniques.
Questioning the Status Quo:
Not being bound by social conventions, Shaun often questions the decisions of his superiors if he believes they are not logically correct for the patient's well-being.

Shaun possesses a near-perfect memory, extraordinary, genius-level intelligence, and excellent analytical skills that allow him to make highly accurate diagnoses. He is very perceptive and often expresses his perceptions bluntly, such as when he pointed out that Neil, his superior, was arrogant on their first day together. Notably, Shaun was the only person in the entire hospital who suspected that Dr. Andrews would choose to remain as Chief of Surgery rather than appoint a new one, allowing him to win the bet everyone had made about the decision. His thought process is frequently captured in an on-screen overlay, demonstrating his eidetic memory. This shows that he learns best visually. Over time, Shaun's unique visualization process has become apparent to those around him, to the point that they can tell what he is doing just by looking at him. Claire was even able to realize this after Zack described what Shaun looked like before he fainted. Shaun's unique diagnostic skills are recognized by many, including Dr. Han, who was prejudiced against Shaun because of his autism. Jared expressed his affection for this years after last seeing Shaun, and Asher commented that watching Shaun do it was the highlight of his day.

His autism is evident to many of the people he interacts with. He exhibits several traits of his condition, including heightened sensory sensitivity, abnormal intonation when speaking, a lack of interest in eye contact, and a strong reliance on routine. He also sometimes becomes fixated on certain objects, particularly the toy scalpel Steve gave him. He insists on eating the same breakfast every day: yogurt and a green apple, except on Mondays, when he has breakfast with Dr. Glassman at the hospital. He loves eating chocolate chip pancakes with a glass of milk, and as a child, he enjoyed peanut butter and jelly sandwiches on white bread and chocolate chip cookies. Although Shaun was clearly uncomfortable treating Liam (the first autistic person he met), his unique perspective on the young man's condition allowed him to recognize that he was autistic and not psychotic, as initially believed, to help him calm down for most of the MRI, and to figure out what was wrong with him. He also helped Liam's parents (who initially discriminated against Shaun because of his autism) understand that they had caused their son's condition, and they subsequently began to listen to Liam instead of making decisions unilaterally for him. When a buzzing light and the stressful situation caused Shaun to experience sensory overload and collapse into the fetal position, Morgan helped him overcome it by focusing his attention on the surgery, and Park identified the cause of Shaun's distraction and eliminated the source by breaking the light. When Shaun again experienced sensory overload during a C-section, his first solo surgery, he successfully overcame it and found a solution to the problem.

Despite his difficult childhood, Shaun maintains a generally positive outlook on life and chose to study medicine in the hope of improving the lives of others. However, after the death of his rabbit and younger brother, he became reluctant to form deeper relationships, which possibly contributed to his isolation. Shaun is fluent in Spanish and knows eight words in Yoruba (two of which he learned from Jared), though he still struggles to communicate in any language he knows. He has also mentioned his interest in soccer at least once, as well as the TV series "Starsky & Hutch." Shaun's dislikes include pickles, mushrooms, anchovies, olives, garlic, mayonnaise, nuts, Chinese food, cinnamon bread, dried fruit, seeds or berries, the effects of coffee, confrontation, and direct questions. He dislikes physical contact unless he initiates it or it comes from people he feels very comfortable with. It is shown that he likes cats, fish, and rabbits, but not so much dogs until he forged his own bond with Buddy.

Shaun's struggles with empathy and communication led him to tell Diane Monroe, while treating Persephone, that Diane's antidepressants may have caused Persephone's severe birth defects—something no other doctor would ever have said. However, Shaun's candor has sometimes proven helpful; he pointed out Suzanne's misgivings about Alex staying with her after his cancer surgery, prompting the couple to confront those misgivings and work through a solution. Carly even found Shaun's candor and obvious concern for his patients endearing when they first met, a stark contrast to many people who dislike such openness. She and Lea have also come to believe that Shaun is the most genuine and honest person they know. While treating Mitchell Stewart and Isla Liu, both wanted the truth, and they knew he would give it to them.

Shaun's communication difficulties prevented him from picking up on the cues that Zack Cordell was angry with him at the bar, which further enraged Zack when Shaun said he didn't like being touched. As Glassman pointed out, Shaun, despite his cruel and violent actions, showed mercy toward Zack, which is remarkable in itself. Even more remarkable was Shaun's assertion that Zack had been misdiagnosed before collapsing from his injuries led Claire to successfully replicate Shaun's thought processes to see through his eyes and save Zack's life. In doing so, Shaun made Claire a better doctor through the inspiration he provided. According to Glassman, this demonstrates that Shaun does communicate (albeit not in the way others might), makes himself heard, and inspires others. Shaun also inspired Andrews—who was initially the most reluctant to hire him—to stand up to Han and fire him so that Shaun could be rehired, offering Andrews advice that Steve had once given him. Claire even considered Shaun a role model in some ways and sought his advice on how to deal with her mother.
