Trepan was created in Nova Cronum and belonged to the Normal class. His alternate form was a car. Before the war on Cybertron, he was working at the Institute's "Asset Abandonment" clinic somewhere near Transquanta Heights when a forensics team led by Prowl and Science Corps member Wheeljack arrived to investigate a murder in the Heights. Despite Trepan's charm and denial of ever seeing any of the suspects, Wheeljack noticed a tattoo on his wrist. Trepan tried to hide it, but under pressure, he was forced to admit that it read "purification and control," despite claiming he had previously worked in sanitation. A police officer immediately handcuffed him, and the team discovered the premises beneath the clinic. When they returned from the investigation, Trepan had escaped.

Trepan was soon sent to Messatin to make Megatron more obedient. He was forced to stop at the last second when Rung arrived, creating political complications.

Following the collapse of the Senate at the start of the Great War, it is unknown what happened to Trepan and the other members of the Institute, though most believe they died.

Trepan is a mysterious Transformer. He appears to work at a child-abandonment clinic, but he's actually an employee of a mysterious institution known as the Institute, which practices mnemonic surgery, shadow play, and who knows what else in the field of mind-bending. As a member of an underground organization, he should probably be more careful about what he puts on himself.
