Doc Hudson's character is based on a real 1951 Hudson Hornet, from which he received his name, number, and paint scheme.

During his Piston Cup racing days, Doc wore a dark blue paint job with chrome bumpers and grille. His door panels featured a yellow number, "51," and the "Fabulous Hudson Hornet" script in white and yellow. Twin-H-Power lettering appeared on both sides of the hood, with the Piston Cup logo underneath. During his racing career, Doc rode on red wheels, but official images suggest he upgraded to chrome at some point.

When Doc arrived in Radiator Springs to open his own medical clinic, he tried to hide his past by removing most of the stickers and leaving only the dark blue paint, chrome grille, bumper, and trim.

When Lightning McQueen arrives in Radiator Springs and accidentally crashes the main road, Doc initially reacts extremely negatively, as the rookie's race car reminds him of his past and the racing world he left behind. Hesitant to run McQueen out of town, Doc sentences him to community service repairing the road, which initially causes tension between them. It was then that Doc revealed his tragic past to McQueen, explaining why he initially wanted him out of town and why, in the second half of the film, he refers to his trophies as "a bunch of empty cups." However, at one point, Doc attempted to befriend McQueen and offered him sound advice on how to make the most of the mud, but McQueen ignored it. When McQueen decides to stay in Radiator Springs after fixing the road, Doc decides to get rid of him by tipping the press of his whereabouts. He believes it's best for everyone, though in reality, he's only doing it to satisfy his own ego after being scolded by Sally. However, seeing his friends upset by McQueen's departure made Doc realize how important and beloved McQueen had become to them. Doc then makes amends by telling his friends about his former racing career and becomes Lightning's chief mechanic to support him in the race, despite realizing the race could go wrong thanks to Chick Hicks. Doc was later impressed to learn that McQueen had given up the Piston Cup to Chick Hicks so he could help The King complete his final race. Doc learned his lesson, which he taught McQueen when the latter was forced to retire from racing after an accident.

By becoming McQueen's mentor, Doc showed who he really is: a caring, supportive father and grandfather who has finally come to terms with his past and the sport he loves.

In the film, he acts disrespectfully toward Lightning McQueen after his first win at the Dinoco 400 at Copper Canyon Speedway, and later taunts Cruz Ramirez during the Florida 500. As Cruz begins to climb several spots and moves into third place, he slows down to stay close to her and provoke her. Near the end of the race, an impatient Storm snaps at Cruz after she cuts him off, crashing into the wall of the speedway. His attitude toward people led him to be quite withdrawn and solitary, which became apparent when his agent, Harv, gave him some tickets to distribute to "friends," unaware that Lightning had no friends. Even with his henchmen, Chick is extremely rude, arrogant, and self-centered.
