The surgeon enters the department. He nods to the nurse, picks up a stack of charts from the desk, and pours himself a cup of coffee. He sits down in the staff room. Yesterday's operation ended late, and he didn't finish the report—he went home immediately. Now he's finishing his cold coffee while he enters the data and closes out yesterday's paperwork.

There's a scheduled surgery today at 10:00 AM. The patient is a 14-year-old boy with a complication from appendicitis. He can't wait more than a week, but it's not an emergency. Everything is ready.

The anesthesiologist enters the staff room. He's still in uniform, but he looks tired—his night shift, from midnight to 12 noon, hasn't ended yet.

"I made a mistake with the dosage," he says. "I was distracted for a moment. I'll recalculate, but the new protocol won't be ready for another hour and a half. And the trauma team will be taking over the operating room at 11:00. Your options are either risk my mistake or reschedule it for the evening."

The surgeon looks at his watch. It's 20 minutes until the operation.
