Chapter 1
Iroha, after having recurring dreams about a girl in a hospital, arrives in the Shinsei district of Kamihama to discover the girl's identity. She encounters a large number of witches and familiars, more than in her hometown of Takarazaki. Upon entering a witch barrier, she saves Kaede Akino, a magical girl, from being attacked by familiars. Kaede warns Iroha about the witches' growing power in Kamihama and mentions having seen a "Mini-Kyubey." Iroha searches for the Mini-Kyubey but is ambushed by familiars and loses consciousness, having visions of a blue magical girl and waking up in a park with Momoko Togame, another magical girl. Momoko offers to help Iroha and takes her to Mitama Yakumo, "The Coordinator," who can strengthen the magic of other magical girls. Despite Mitama's initial reluctance to help without payment, Momoko offers to cover the cost. Mitama begins to "adjust" Iroha's Soul Gem, allowing her to have a clearer view of the hospital room.
Later, Iroha, Momoko, and Yachiyo locate a witch's barrier and suspect she fled upon sensing their presence. Yachiyo challenges Iroha to prove her strength, despite considering her weak. Inside the barrier, Iroha stops upon seeing the Mini-Kyubey and attempts to communicate with it, while Momoko reminds her of her agreement with Yachiyo. The Mini-Kyubey guides the girls along an alternate route to the witch Zenobia. Momoko encourages Iroha to defeat the witch so she can be recognized as a "Kamihama girl." Iroha is unexpectedly attacked by Yachiyo, but is then given the opportunity to...

One night, Iroha goes to Satomi Medical Center to save Ui, Touka, and Nemu from the witch Mapycя, who had infiltrated the center and was heading for the girls' room. Iroha manages to rescue them just as they are attacked by the witch's familiars. Kyubey, who is with Iroha, tells her where the witch is going, but Iroha is more worried about her "little sisters." While Iroha tries to calm them down and promises to protect them from the witch, Touka recognizes Kyubey when Iroha calls him by name. The witch then attacks them, but Iroha protects her three little sisters and manages to defeat Mapycя with difficulty.
A couple of days after this event, Iroha would forget about her sister Ui, Touka and Nemu, for a couple of weeks, thus beginning the story of Magia Record.

Shortly after Iroha signed the contract, Ui recovered from her illness and was discharged. To celebrate, Iroha decided to prepare some food for Ui that she hadn't been able to eat before, with the doctor's permission. Ui asked Iroha for a hamburger, even though she didn't yet know how to make them. Later, Iroha made the hamburger patty at home. Since she had leftover ingredients, she decided to bake a cake as well, but accidentally burned the hamburger. Once back at the hospital, Iroha presented the cake to Ui, but then her mother revealed that she had also brought Iroha's hamburger. Iroha asked her mother why she had brought the hamburger, and her mother replied that it was because Iroha had gone to the trouble of making it for Ui. Despite Iroha's concerns, Ui enjoyed her meal, much to Iroha's delight.
Desire
One day, Ui's health deteriorated to the point where she was near death. It was then that Kyubey appeared before Iroha and offered her a contract. Iroha made her wish: "Please heal Ui." And so, Ui regained her health just as she was about to die, while Iroha became a Puella Magi and began fighting witches.

Touka and Nemu tell Ui to draw pictures about their stories, since she's the best at it. Ui draws the breaking ladder, the shrine horse, and the anonymous AI girl (Ai). These images become the beginning of a new map. Eventually, the girls ask Iroha to invent her own story, and she comes up with the story of the Eternal Cherry Blossom, drawing the tree and the four chairs where the four of them would meet. When Iroha finishes telling the story of the Eternal Cherry Blossom to the girls, all three love it because it inspires them to keep going. Nemu loves it so much that she decides to write it down in her notebook to develop the story a bit and give it some more background. Then, Nemu narrates the story as one she heard from her beloved older sister, with some details added by herself:

"Somewhere in the forests surrounding Hokuyo District, there is a large grassy field big enough for everyone to run around, and a large cherry tree where everyone can sit and watch the blossoms beneath it. One day, when the three girls from the hospital have recovered and gone out into the world, they and the girl who always came to visit them will meet there again. Then, that big cherry tree will bloom in all its splendor, celebrating the happy reunion of the four girls."
Finally, Nemu writes the names of the four girls into the story, thus giving rise to the basis for the Eternal Cherry Tree rumor.

Iroha visits Ui at Satomi Medical Center, where Ui shows her a map of Kamihama. The girls explain that, due to their hospitalization, they don't know the city well and want to explore it beyond the hospital. Despite not being from Kamihama, Ui and her friends Touka and Nemu are excited to discover new places in the city, based on recommendations from other patients and hospital staff. Iroha, who isn't a local, contributes to the map with suggestions, filling it with pins and photos, though they soon realize they've exhausted all the available stories about the city.

A few days later, Ui asked Iroha to bring her a sketchbook, but Iroha didn't know why. Upon arriving at the girls' room, Ui said that they had heard so much about places in Kamihama that they wanted to think about what might be there—imaginary things like stories or urban legends. Therefore, according to Nemu, they would try to write their own imaginary Kamihama. Ui added that it would be their farewell letter to the future. The girls then began to imagine stories and urban legends with rumors that supposedly happened in Kamihama. Touka said that she had heard about a rule for breaking friendships and the dangers of doing so, similar to when she and Nemu fought too much and broke their friendship. This gave her the idea of "a shadow girl on a staircase who will kidnap you if you try to rekindle the friendship" (similar to the Rumor of the End of Friendship Rule). Ui comes up with a rumor in which she meets someone she wants to marry at the Mizuna Shrine, drawing the Horse that "will reunite her with her future husband" (The Rumor of the Shrine of Invocation). Nemu imagines a radio world, with information about an anonymous abandoned person (The Rumor of Anonymous).

During Iroha's visit to the hospital, Ui questions Touka and Nemu about their constant bickering. Nemu, quoting French author André Maurois, expresses that their friendship allows them to be authentic and that she only feels complete with them, considering them her family. Touka finds humor in the idea of "best friends as family" due to the statistical improbability of such a bond, citing a study that indicates the probability of finding even one best friend is only 0.00000004%. After being accused by Nemu of being insensitive in quantifying their friendship, Touka gets into an argument with her, much to Ui's annoyance.

Nemu tells Iroha and Ui about her new story. Ui tells Iroha that Nemu's new tale was as amazing as the others, and Iroha agrees, noting that it filled her with warmth. Nemu smiles and says that seeing Iroha and Ui enjoy her story is "the ultimate joy." Touka asks if they really liked the story, as she didn't understand what made it so good. Iroha asks why she says this, to which the girl replies that "it was all just fantasy" and that it was "intense and unrealistic." Nemu warns Touka that if all stories were judged the way she does, great advancements throughout history would be missed. She also points out that people like her have enabled people like Touka to imagine things that didn't yet exist but could one day, and that they bring happiness to others.

Iroha grew up alongside her younger sister, Ui Tamaki, in Takarazaki City. Iroha was only close to her younger sister, as she never really had any true friends to become close to. However, at one point, Ui fell ill, and Iroha had to take care of her since their parents were always away working. Iroha learned to cook for Ui, but eventually, Ui was admitted to Satomi Medical Center in Kamihama City, a neighboring city to Takarazaki. It was there that Iroha and Ui met Touka Satomi and Nemu Hiiragi, two other girls with the same illness as Ui, with whom Iroha ended up sharing a room in the hospital. Because of all the time Iroha spent at the hospital, the four girls became very close, and Iroha also became like an older sister to the other two girls, who began calling her Onee-sama/san ("older sister" in Japanese).
