Springtrap is a worn-out version of Springy Bonnie, an anthropomorphic rabbit animatronic. His once-yellow body has been painted a swampy green and is riddled with holes, wires, and endoskeletal fragments. The animatronic's main feature is the body of William Afton, who became trapped in Bonnie's suit and became imprisoned within it, becoming what is known as Springtrap.

Phantom is an animatronic in the form of a boy with olive skin and short dark hair. He has a fairly stocky build and is small in stature. Like other Toy Animatronics, the robot's body is made of shiny plastic; all its parts fit tightly together. His entire body is covered in soot stains, forming tapering stripes resembling scratches, often obscuring the light stripes of his shirt.

The phantom has short, thin limbs, tightly attached to its round body. The animatronic has no fingers: its hands are spheres with holes for holding long, thin objects.

He has a cartoonishly large, round head with no neck. Instead of a neck, he has a white bayonet. He has large white pupils with gray irises, like phantoms, rosy green hemispheres that imitate blush, and a triangular, dark green nose. Balloon Boy's mouth is open in a smile, revealing small, square teeth: five on top and three below.

In terms of accessories and clothing, he wears a striped red and blue shirt, blue pants, and brown boots, as well as a propeller hat and two gray buttons with holes located on his shirt.

His version in Five Nights at Freddy's: Help Wanted differs from the original: he lacks a shine. The red stripes on his shirt are now green. He has no legs. He has multiple holes in his frame. He now has a nameplate stained with black dirt. He has no nose. He has also darkened.

A phantom version of Mangle, Phantom Mangle is a phantom anthropomorphic animatronic fox with a dirty, dark green coloring and a broken state: she is depicted with a steel endoskeleton in place of a hull-covered body, a severely damaged anatomy, and wires protruding from everywhere. The only parts of her shiny body that remain are her head, hands, and feet.

The phantom's limbs are composed of a variety of endoskeletal parts. Some of the parts used to construct arms and legs in full-sized robots have entirely different purposes in Phantom Mangle. For example, Mangle's left shoulder contains an unused appendage, while a second head grows from her right. Where a simple animatronic's neck would be, a wide white hand with four jointed fingers is attached. Three legs and an arm, clearly composed of a shin and thigh, grow from the pelvis. Only one of the feet is finished in a plastic-like material, while the others consist solely of internal skeleton; each foot has three toes. A light-colored tube, resembling a tail, is also attached to the back of the pelvis.

Phantom Mangle's torso is permanently warped and devoid of plating, consisting of a spine-like core and three flat ribs on each side. In some places, the endoskeleton's torso is studded with rivets and small steel inserts resembling round buttons.

One of Phantom Mangle's distinctive features is the presence of two nearly identical heads, one of which is covered by a suit, while the other is not. The head, encased in a shell, is round, attached to a makeshift neck made from several arm fragments inserted into the chest, and is covered in numerous soot stains that form tapering stripes resembling scratches. On top of the head are pointed fox ears, attached to flat rods, from which also extend tufts of dark red and green wires. Phantom Chica is an anthropomorphic yellow chicken animatronic, very similar in appearance to Chica. The character's body is made of matte fabric, covered in dark, stained areas. Her entire body is covered in soot stains, forming tapering stripes resembling scratches.

The Phantom has a rounded head, shaped like an apple. On its crown is a tuft of three small, teardrop-shaped fragments arranged in a ring, widening toward the top, imitating feathers. The chicken's face has two widely spaced, circular eye sockets, within which are deep-set round eyes with gray irises and glowing white pupils, framed by black, glossy metal eyelids. Above the animatronic's eye sockets are flat, arched eyebrows, widening toward the center. Below, in the center of the antagonist's face, is a bulky, oval upper half of an orange, pyramidal beak with a rounded edge and two slits at the base. Beneath the upper portion of the beak is its trapezoidal, rounded lower half. On the inner edge of the lower fragment are fixed ten smoothed parallelepiped teeth of white color, with a large gap in the middle.

Chica's head is connected to the torso by two metal endoskeleton neck bayonets with curved metal cable rods. Chica's oval torso, slightly tapering toward the top, is a solid mustard-yellow color. A round white bib is placed on the animatronic's chest, extending from the shoulder area and beginning behind the robot's back. It is decorated with a confetti print in shades of red, orange, yellow, green, blue, and purple. Above the pattern, in the center of the bib, are the words "LET'S EAT!!!" in bold yellow font, surrounded by a thick purple outline.

Freddy's hands have only four fingers. The bear is completely missing his right shin, and his hips and shoulder joints have ragged holes from which torn blue and yellow wires protrude.

Phantom Freddy's torso is shaped like a pear. The animatronic has a large, round belly with a dark spot in the middle.

Phantom Freddy has a round head with no sign of a neck. On top of his head is a small bear ear, attached to a flat bayonet on the right; in place of the left ear are tattered white, blue, and red wires. The robot has a large muzzle with a shiny, oval, black nose. Inside his mouth, on both jaws, are rows of large, white, square-shaped teeth. The lower jaw is straight and separate from the top of his head. Freddy has empty black eyes with gray irises and glowing white pupils. Above them hang thick, black eyebrows, widening at the edges. Tufts of tattered wires also protrude from his eye sockets.

Like his real-life counterpart, Phantom Freddy wears a black top hat, two round black buttons, and a matching bow tie. In his right hand, he holds a mock-up microphone with a light-colored body.

The Phantom Puppet is a phantom created by the guard's subconscious, but is based on a real animatronic—the Puppet. The Phantom Puppet is a doll-like animatronic, resembling a humanoid creature with eerily distorted features. Its body is simplified, thin, gaunt, and pitch-black. However, the Phantom Puppet is quite tall, almost as tall as the other animatronics, if not taller.

This animatronic has very long arms, with numerous darkened and burnt glossy stripes on its forearms and calves. Like other body parts, the hands are greatly simplified in design compared to a real person or other robots. The Phantom Puppet has a rounded chest and hips, as well as a thin, elongated waist. Three decorative white buttons with four holes drilled into them are still attached to the doll's chest.

It has a long neck and a small, featureless black head. It wears a charred white mask with no nose or ears, but narrow eye slits and a large mouth, open in a wide grin. The makeup on the creepy puppet's mask resembles a circus clown or mime: it has no hair, eyebrows, or eyelashes, but does have dark lips and cheeks. Most of the time, the Phantom Puppet's eye sockets act as Springtrap's pupils.
