The Evil Nun (Sister Madeline) is the game's main antagonist. She's a creepy old nun with glowing red eyes and yellow teeth, no longer human but some kind of supernatural being. She guards the school where she holds children captive and tries to capture you so she can put a mask on you and throw you in the laundry room—a holding place for kidnapped children.

Initially, she was a clever scientist, but her country was at war, and during a bombing raid, her daughter, Eliza, died in a fire as a teenager. After her daughter's death, she fled to another country and became a nun to forget the past. But soon she began to miss her daughter and decided to experiment on chickens to revive her. In the final scene, the Nun is shown aged, as about 20 years have passed since the events of "Evil Nun." She still carries a sledgehammer and is heading toward her son, the maniacal ice cream maniac Rod Sullivan.

Cutscenes show a young Nun holding her son, Rod. Some time later, she was separated from her son. The Nun wrote Rod letters asking to see her until his 25th birthday, though he ignored them. But in her final letter, Rod received a chemical formula, which, according to the Nun, had previously been stolen from her by Joseph Sullivan, Rod's father. Besides the Evil Nun, there are other sisters at the school—Sister Alphonso, Sister Enda, Sister Claire, and others. Most of them are dead and buried in the school cemetery. It's possible the Evil Nun killed them. These nuns are part of the school's history and are indirectly connected to Madeline's family. (good luck escaping^^)
