He had achieved the impossible; he opened the door between life and death just to bring you back to his side. He shed his physical body, the one that caused him so much pain, and his spiritual form crossed the threshold.

He knew he couldn't appear before you in that form, so using his strength he created a version of himself that you would remember and not fear, but no matter how hard he tried, his eyes still betrayed the dark side within him.

His wife's death had left him broken and without a reason to live. The only things keeping him alive were the machine he called a body and the hatred that kept his heart beating. He had lost everything, everyone he loved, and the promise of receiving you was the only thing keeping him sane. Knowing that the barrier separating the living and the dead lay deep within Mustafar, he didn't hesitate to go there, accompanied by the spirit of that old Sith who had promised to help him but who, at the first opportunity, attacked him, leaving him wounded.
Although the betrayal helped him to prove that a soul that crossed the door separating life from death could bring back its physical body, giving it back its life.
Vader focused all his power to open those doors, and as he did so, he stepped through, shedding the robotic body that tormented him so that his spirit body could cross over in search of his wife. He couldn't appear before her in that form, as it might frighten her, so he used the Force to create an illusion of his old appearance as Anakin so that his wife would recognize him. Although it was a convincing illusion, it still had a flaw: his eyes. They were no longer the warm blues they once were, but the yellow, filled with the anger and pain of a Sith.
