Guts is the protagonist of Berserk, a dark and violent story set in a medieval world rife with war, corruption, demonic creatures, and sinister magic. His life is marked by suffering from the very beginning: he was born from his mother's corpse hanging from a tree and raised in an abusive environment, taken in as a child by the wife of Gambino, the leader of a mercenary band. This forced him to learn to fight from a young age to survive, even though Gambino hated him. When Gambino dies, he escapes to fight on his own.

He eventually joins a mercenary group called the Band of the Hawk, led by Griffith, a charismatic strategist with dreams of power. Guts and Griffith develop a complex relationship, a blend of friendship, admiration, and rivalry. During this time, Guts finds purpose and a sense of belonging, and he also falls in love with Casca, a strong and important warrior within the group.

However, everything changes with the story's pivotal event: "The Eclipse." Griffith, obsessed with achieving his ambition of having his own kingdom, sacrifices the Band of the Hawk in a dark ritual to become a demonic being known as "Femto," one of the five members of the "God Hand," supernatural entities that manipulate human destiny. Guts and Casca survive, but she is deeply traumatized, so much so that she loses control of herself and is left with the mind of a young child. Griffith then destroys everything that meant anything to Guts.

From that moment on, Guts embarks on a quest for revenge, pursuing Griffith and facing horrific creatures known as "apostles"—humans who sacrificed their loved ones to gain demonic power. He is also marked with what is called "the mark of sacrifice," which constantly attracts evil spirits and monsters, forcing him to fight relentlessly.
