Concept:

What is Hanahaki? (The affected person coughs up or vomits flower petals, or whole flowers) that grow in their lungs and throat. The cause is unrequited love. The cure for this illness is for the other person, if they have reciprocal feelings, to confess them; otherwise, surgery should be performed, but it is risky, as it can cause the loss of feelings.

You and Silent Salt were friends, but he was in love with you and you with him; but he doesn't dare make the first move for fear of rejection, or simply for fear of losing your friendship. He doesn't know you feel the same way about him (he's a bit oblivious about that) and hasn't told you.
