The Doctor shows interest in the protagonist from their very first interaction. However, his interest wanes upon seeing Pierrot's red ticket. The reason for this is unknown, but it is speculated that he felt disappointed or jealous that someone else had gotten there first.

The doctor most likely admires him and considers him his leader.

Currently, no canonical interactions have occurred beyond the external material. Therefore, its dynamics remain unexplored.

The Doctor gets along surprisingly well with Harlequin. Unlike the others, he is rarely mocked by him, and Harlequin seems to feel safe in his presence. The Doctor is able to see through Harlequin's facade.

The Doctor is shrouded in mystery. Little is known about him within the context of the game, nor about his relationship with the characters. He is reserved, composed in almost any situation, and prefers to speak in simple sentences when addressing the protagonist in their first encounter. He may speak with little regard for social norms, but he understands to some extent how to calm the protagonist while administering an injection to suppress their urge to fight. Although distant, he doesn't necessarily avoid expressing emotions; he simply displays them in different ways.

The Doctor exhibits two distinct facets to his personality: one calm and serene, and the other passionate and sadistic. The shift between these two is visually apparent when his violent impulses take hold, with red flashes in his eyes. Regardless of which facet he displays, he is an intimidating and intelligent man who prefers cunning to brute force to get his way. He is responsible for some form of human experimentation within the circus, though the details of what experiments are conducted and for what purpose remain hidden. His specimens can be seen throughout his tent, their eyes glowing the same cyan hue as his own.

The Doctor is seen wearing a long, structured coat in crimson and black, fastened with neat rows of gold buttons; his style is reminiscent of plague doctors from our distant past. A beak-shaped mask further accentuates the resemblance to the Black Death doctor, with two cyan eyes that allow him to see through them. These eyes flicker red when he displays his violent side. The Doctor wears crimson gloves that end in sharp claws dripping with dried blood. He wears a floor-length cloak, red on the outside and cyan on the inside. From beneath the hood, a tuft of hair may emerge, matching the crimson of his attire.
