Itsuki is a serious girl with long hair. Her hair color is orange-red, ranging from light pink to salmon pink in Volume 1, and a deep orange that turns red in Volume 4. Her most notable accessories are a pair of star-shaped hair clips near her eyes, which Fuutarou once called "old-fashioned." Like her sisters, she has dark blue eyes, is of average height, and has a good figure.

She wears glasses while in class or while studying. Her classmates often say she looks pretty.

According to Shimoda, she looks a lot like Rena Nakano (her mother). She wears her uniform in a simple way, without any accessories, and like most of the students, she carries a handbag.

She is the heaviest sister, though she refuses to acknowledge it. Itsuki is a serious girl, usually waking up early compared to her sisters. She also has excellent manners, to the point that she is very particular about them. She has never been late or missed a class, and always speaks politely.

Itsuki has been shown to be a girl who holds grudges, as demonstrated when she rejects Fuutarou Uesugi's teaching, partly due to their previous argument. She also struggles to be honest with herself and has difficulty being more direct. Both of these qualities often lead her to struggle alone. Itsuki is usually overly precise about cooking times.

Itsuki can sometimes be a crybaby, whether it's due to unfavorable situations, terrifying things, or her stubborn nature.

Itsuki has a huge appetite and is mostly seen eating throughout the series. He doesn't seem to mind treating his sisters unfairly when it comes to food. When Itsuki suggests sharing his cakes, Ichika suspects his real goal is to try all the flavors.
