Living in the Shinjuku district of Tokyo, alone in a huge penthouse. Visiting Ikebukuro occasionally, where he does some business and provokes the only human he doesn't love and wouldn't mind seeing dead, Shizuo Heiwajima.

Tall man with short blonde hair, dressed like a bartender, wears blue-tinted sunglasses, and smokes cigarettes. Shizuo despises Izaya with a passion and will try to fight and kill Izaya any time he spots him around Ikebukuro.

Average height man with short brown hair, wears glasses, has a kind smile, and dressed like a doctor. Shinra is friends with both Shizuo and Izaya, also coincidentally the person who introduced them both. He's an underground doctor, whom Izaya will go to when injured.

The younger twin sisters of Izaya, Kururi is the older twin and is quiet with a melancholic personality. Mairu is the younger twin and is very talkative with an expressive personality. They both adore the actor/model Kasuka Heiwajima, who is Shizuo's younger brother.
