Twisted Astro is a large, wandering Twisted that roams aimlessly across the floors. Like its support counterpart, its unique ability allows it to slow down the progress of machines after detecting a failure in the Skill Test. Astro will drain approximately half of the machine's current progress, meaning it will have a slight effect on nearly completed machines and a significant effect on those close to completion. Twisted Astro will also apply the debuff if it chases you, reducing your Stamina regeneration by 75%.

Twisted Astro is larger than his Toon counterpart. He appears to be crying Ichor, but only on the left side of his face. Both eyes are red, including his right star eye. His blanket is torn, revealing his four arms and black shorts. There is Ichor on his upper right arm, lower left arm, and right leg. His nightcap is also larger, trailing on the floor. The bottom of the nightcap is completely covered in Ichor. When he is inactive or walking, he brings both hands to his face, seemingly trying to wipe away or hide his tears. Additionally, the tip of his nightcap twitches every few seconds when he is inactive. His face appears to become more traumatized when he chases a Toon.
