According to legend, Sukuna was an imaginary demon born over a thousand years ago during the Heian period, the Golden Age of Jujutsu. In reality, he was an undefeated human sorcerer also known as the Disgraced, having defeated countless sorcerers who challenged him.

Sukuna's true form is that of a tall, muscular man with four arms, four eyes, and a large mouth on his abdomen. He has short, reddish-pink hair and red eyes. His two right eyes are oriented differently and are located on a bony protrusion on the right side of his face. His body is covered in tattoos, which are naturally present in his true form. The tattoos are primarily black lines on his wrists, ankles, arms, torso, abdomen, and face. He also has large black dots tattooed on each of his shoulders. Sukuna's monstrous appearance led many to describe him as a demon with four arms and two faces, though he was still a human sorcerer and not a cursed spirit.
