Makoto is a tall young man with pale, yellowish skin, slanted gray eyes, and black hair combed back, slightly disheveled, with some strands falling over his forehead.

After receiving his first OBE decoration, being "purified" by Takahito Komagusu, and having his hair trimmed a little at the ends to give him a more presentable appearance, he cut all his hair off, leaving it quite short, without combing it back, but still with a disheveled look.

In most of his appearances throughout the series, he wears a loose white t-shirt under an open, long-sleeved black jacket with chest pockets, paired with light gray pants and black sneakers.

Makoto is portrayed as a cold and collected delinquent who only attends classes and shows up at school when he feels like it (which is rarely). He is distant and arguably antisocial, never seeking contact with those around him.

When he was very young, his mother dropped him, causing a severe head injury that resulted in psychological trauma and slightly distorted his morality. He also becomes easily enraged by certain topics brought up around him. When he encounters Masayuki Nakajima and Taro Komori near the shrine and Masayuki provokes him by mentioning Makoto's father, he grabs Masayuki by the neck and holds him up on the stairs, threatening to drop him backward, stating that he "has no qualms about killing people."

Later, when the three of them first explore the abandoned hospital, he also mentions that he didn't understand why it was wrong to kill people.

When Makoto was about four years old, after Taro and his older sister were kidnapped, the residents of Suiten Town were questioned to find out if they knew anything about the kidnapping or the whereabouts of the abducted children. Makoto's grandmother was questioned by the police, and shortly after she gave her statement, her father locked himself in his room. Upon opening the door, Makoto noticed the scene, as he seemed traumatized by the event, since it's the first thing that comes to mind when traveling with Masayuki and Taro.

A few years later, when Makoto was seven years old, his mother abandoned him, leaving him in the Ōgami house with only his grandmother and a sinister-looking woman.

Episode 1
Makoto makes a brief appearance in the schoolyard while Taro is in his class, and the latter's classmates make comments about his bad reputation and his tendency to skip class and even entire days of school.
