Hisagomaru's physical manifestation takes the form of a small, white, robot-like figure with a cylindrical torso and a spherical head. He has thin arms and legs with flat hands and feet, a rectangular mouth, and a round, completely black right eye. Instead of a left eye, Hisagomaru has a long, vertical meter with several measuring marks that extends across almost his entire face.

Hisagomaru is very shy, hiding in the sewers beneath the Seireitei and fleeing upon encountering other living beings. He is also extremely apologetic, repeatedly and profusely apologizing to Hōzukimaru and Nanao Ise instead of answering their questions. However, he can be very courageous when called to action, and single-handedly defeated the Tentacle Tōjū when all other combatants had already been defeated. Furthermore, Hisagomaru absolutely refuses to harm any living being for food.

Hisagomaru has no problem with Hanatarō Yamada and repeatedly tried to return to his sword after Muramasa manifested it, but believed it had been abandoned when he failed to do so.

Hanatarō Yamada (山田 花太郎, Yamada Hanatarō) is the co-3rd seat and former 7th seat of the 4th Division in the Gotei 13, serving as the leader of the 14th Advanced Relief Team.

When he was the 7th Seat, Hanatarō had a poster in his quarters saying "Aim for the 6th Seat". Hanatarō shares a birthday with Keigo Asano and Mashiro Kuna.

Like all other Shinigami, Hanatarō possesses a Zanpakutō, although he is rarely seen carrying it. Since his abilities lean more towards medical support than combat, Hanatarō tends to lose his Zanpakutō, as he rarely needs to use it.

Hisagomaru (瓠丸, Gourd) is a katana with a blue hilt and a circular guard with a brown sphere or pouch attached to it. There is a gauge running along the length of the blade.

Healing: Hisagomaru, unlike a common Zanpakutō, does not cause damage when the blade hits a target, but rather heals the wounds of anyone it touches. Upon contact with a target, a red smoke is emitted from the wound as it heals. The red smoke then enters the blade, causing its meter to fill with a red light consistent with the level of the wound. Minor wounds will barely fill the meter, while Encroachment is large enough to fill it instantly. Shikai: Akeiro Hisagomaru (朱色瓠丸, Red Gourd) Hisagomaru's Shikai command is "Fulfill" (満たせ, Mitase; "Fill" in the English version). When the bar is full, Hisagomaru automatically initiates its Shikai and transforms into Akeiro Hisagomaru. In this form, it takes the shape of a scalpel.

Specialist Healer: As a Shinigami of the 4th Division, Hanatarō possesses almost no combat skills, his only true weapon being his Zanpakutō in its released form. However, he is skilled in the healing techniques he learned in the 4th Division.

Spiritual Power: Hanatarō possesses sufficient spiritual power to serve as the 3rd Seat of his Division.

Kidō Practitioner: He is proficient in healing through Kidō.

Kaidō (回道, Turn Way): As the 3rd Seat of the 4th Division, Hanataro is highly proficient in Kaidō, using it to heal injured Shinigami in the line of duty. During his time as the 7th Seat, he healed Renji Abarai from near death so he was able to run to Sōkyoku Hill without problems in a short amount of time. Expert Guide: Because the 4th Division's combat capabilities are very weak compared to the other divisions, they are frequently assigned to perform cleaning tasks in Seireitei, such as sweeping the sewers and prison cells. Because of this, Hanatarō and other members of the 4th Division are very familiar with the sewer system and other secret passages and possess the keys to most buildings.

4th Division Medical Kit: As a member of the 4th Division, Hanatarō possesses remarkable healing powers. Instead of his Zanpakutō, he usually carries a 4th Division medical kit on his back. It is presumed to contain medical supplies, including ointment, bandages, and rejuvenation pills. One of the known items in this kit is Shinten, a green liquid that can knock out anyone with weak spiritual power with a single drop (though it was still potent enough to knock out Yumichika, who is as strong as a lieutenant). It is presumed to be used as an anesthetic. He is also known to carry a type of large pill with a skull symbol, a supplement exclusive to the 4th Division that restores stamina. While the pill given to other members of the 4th Division works as expected, Hanatarō's is merely a flour-filled placebo, given to him as a prank by his seniors, though it appears to be genuine.

He tends to be clumsy and easily tricked. Hanatarō's gentle nature and nervousness make him the target of many bullies and pranks. In fact, he claims to be "the most bullied child in the world." Hanatarō's birthday is April 1st, April Fool's Day. As a result, people spend the entire day playing pranks on him. According to Kon, he has a "plain appearance, but somehow popular with girls."

Despite his cowardly appearance, Hanatarō demonstrates great courage at times. After hearing Rukia Kuchiki's story, he attempts to rescue her, despite his recent criminal record and noble lineage. After meeting Rukia's friend, Ichigo Kurosaki, Hanatarō decides to help him save Rukia, knowing that if caught, he would be imprisoned for treason. After observing Ichigo fight for what he believes in, Hanatarō decides he also wants to take on a more important role, instead of just watching from the sidelines, and begins carrying his Zanpakutō with him.

Hanatarō states, upon introducing himself, that his name is relatively easy to remember, although Ichigo disagrees, claiming that if his name were Hanako (a female name) or Tarō (a male name) it would be easy. According to Ichigo, the name Hanatarō is simply confusing.

Hanatarō is a short male Shinigami with blue eyes and chin-length black hair, with bangs framing the side of his face. He wears a standard shihakushō, with the Fourth Division's medical kit slung over his shoulder.
