His leather jackets included a double-breasted one coloured in black, and a purple corduroy jacket. However, he would sometimes dispense with leather and instead don a plaid print overcoat a yellow duffle coat, or a brown checkered overcoat. His polo-cardigan styles included one striped with shades of blue and orange vertically, a green one with horizontal stripes, a yellow one with a simple orange stripe going down the middle, a golden glow mod-striped design, and a knitted orange v-neck design. Under his jackets, he would wear an orange jumper, a yellow tank top, a red polo-shirt, a white jumper with black stripes, an orange sweater with faint stripes, a white tank top, a white crew-neck shirt, a white v-neck collared top, and retro-styled knitted polo shirt. His leg wear would include pleated checked jacquard trousers, a blue tartan kilt, dark navy jeans with an elasticated snake-themed belt, khaki gurkha trousers, blue denim jeans, moleskin trousers, a brown tartan kilt, brown double-pleated wide trousers with cream leather braces, dark denim jeans, white dress trousers, navy Levi jeans with a brown leather belt, and burgundy glencheck trousers. His socks included knee-high green socks, handknitted marled wool socks, and regular burgundy or beige socks, and his footwear would alternate between Grenson Brady boots, Grenson Fred boots black leather boots, tactical boots, brown high tops, and Stepney Workers Club sneakers.

Standing at 5'10, the Fifteenth Doctor had dark skin, with a "nice" new face which fitted well into place, his hair being short and his nose being "just right". His legs were firm and his eyes were bright and brown in colour. A thin black moustache rested on top his upper lip, and he had a faint scar around his right eye. According to Belinda Chandra, he would frequently have his hair length and style altered by the TARDIS. Melanie Bush described the Fifteenth Doctor as looking "beautiful", and Susan Triad called him a "handsome man". Cherry Sunday likewise found him attractive, as did Maestro and Rogue. A kind woman also said he had a "nice face". The Thirteenth Doctor thought her fifteenth incarnation was "gorgeous” with an "absolutely beautiful face" and a "lovely smile". The Fifteenth Doctor primarily wore a burnt orange double-breasted leather trenchcoat, complete with pockets and belted cuffs. Under his trenchcoat, the Doctor wore a series of zip-up polo-cardigans, with blue trousers and long black socks under a pair of colourful trainers on his feet, which were also coloured in beige, brown, and white. Because he had an enjoyment for dressing up, the Fifteenth Doctor made a habit of alternating his main attire with a variety of clothing, such as switching out his trenchcoat for a leather jacket, his trousers for a kilt, his shoes for boots, and his polo-cardigans for tank-tops and polo-shirts.

he accepted her survival as a victory and expressed his love for her and Belinda as he departed. Not wanting to face the end alone, the Doctor went to the Star of Bethlehem, where he realised he could best describe his life as having been "an absolute joy" before he was engulfed in regeneration energy.

While spending a year at the Sandringham Hotel, the Doctor struck up a friendship with the proprietor, Anita Benn, telling her he made his stay "amazing" as he bid her farewell, telling her people would be "lucky" to know her, and was happy when she settled down to start her own family, though regretted having to learn she had secretly fancied him, feeling he had done her a disservice. The Fifteenth Doctor was open about his attraction towards men, being encapsulated by Ricky September and recognising Christofer Ibrahim as being "a beautiful man." He was particularly enamoured with Rogue, eventually realising that he loved him. Seeing them as made up of "hopes and dreams and wishes", the Doctor thought that children were "the most precious gifts of all", and made it his mission to protect them, breaking down in tears if he realised that a child had come to harm. When describing the process of regeneration to Ruby Sunday, the Fifteenth Doctor told her that a Time Lord would remain the same person despite changing their appearance, even comparing it to taking on a disguise in an opportunity to hide away with a new face. When he realised he could use his own regeneration energy to "jolt reality by one degree" by releasing it into the Time Vortex in order to restore his daughter, Poppy, back into existence, the Doctor bemoaned how much he had enjoyed his fifteenth incarnation, but decided to focus on saving Poppy instead of despairing, even defending his choice as being "one life for a life" that was expensed as a result of saving the world, and deeming it "no world at all" for such a price. Though he was "terrified" that the pain would "eat his soul", and wished he could have had "longer", he was ultimately accepting of giving his own life for Poppy and kept the process at bay in order to ensure she and Belinda were safe. While he was saddened to learn Poppy had been fathered by another in the new reality.

he described himself as being a "visual person," and thus felt the need to see things to aid his thinking process regarding them. The Fifteenth Doctor recalled his third incarnation as being a "great enigma". He professed a love for the Thirteenth Doctor, and was sympathetic about the Fourteenth Doctor's struggles and urged him to retire. When they parted ways, they hugged each other and saluted at one another with affection. He agreed with his predecessor that being separated from his TARDIS would "hurt", and also apologised to his ship after he struck it with a mallet to divide it. He also took umbrage when someone took his ability to control his TARDIS away from him. He remembered Brigadier Alistair Gordon Lethbridge-Stewart as being "the best of men", and maintained a strong friendship with Kate Lethbridge-Stewart, extended that love to the rest of his friends in her UNIT team. He also maintained that he "loved" Mel Bush, considering her to be "bonkers".

The Fifteenth Doctor did not seem to suffer from embarrassment, striding confidently around without trousers after bi-generating, and ecstatically dancing to his own rhythm in a nightclub. He liked to work at his own pace, and would drown out others while on task. He was not above menial labour and worked as both waiting staff and a caretaker to earn money to pay for his stay at the Sandringham Hotel. He could get caught up in his excitement to the point that he risked sabotaging his own efforts and get too into the thrill of explaining a situation to take account of how he was scaring his audience, while also deliberately putting himself in danger if it meant satisfying his curiosity. He could also be a thrill-seeker, opting to detour on his flight to the Rani's Bone Palace to fly under London Bridge and through a Bone Beast that was under fire from a Galvanic beam for no clear reason beyond fun. While he moved to protect bystanders from harm, he was not above criticising their choices or begrudging the circumstances he found himself in. Even though he was dismissive of the policeman that questioned him regarding an accident with an inflatable snowman, the Doctor stopped during his exit to reassure the man that his girlfriend would accept his marriage proposal, because his decision to buy an engagement ring before the sales was proof of character, and happily wished him a "Merry Christmas" before he left. He similarly gifted Trev Simpkins his spare mug after dismissing his attempts to confront him for stealing milk from the Time Hotel. He could also be playfully disobedient, revelling in the chance to commit "scandalous" actions in the Regency era and cheekily correcting himself to "hon" after Cassio Palin-Paleen pointed out that calling an officer like him "babes" was inappropriate. The Doctor embraced his newfound status as a foundling, even proclaiming that it ultimately did not matter where he came from because he was "absolutely lovely" the way he was.

Not without "ice in his hearts", the Doctor would express joy in making his foes suffer if they harmed those close to him. He had little mercy for the "relentless noise" that was caused by those he battled "on behalf of everyday people, who just want to get through their day, and feel safe, and warm, and fed." He got especially exhausted with those who "stamped on the truth, choked the bandwidth and shreded people's patience" by "weaponising lies, taking people's insecurity and fear and making it currency" to "wear them down." When faced with those who endangered lives with such flippancy, he could reach a level of ruthlessness that led him to commit acts of torture on the aggressor, though he would be horrified with himself once he regained his senses. The Fifteenth Doctor took pride in his status as a time traveller, taking offence when Ruby called the goblins time travellers, telling her that their time-surfing capabilities were a mere "bimble" in comparison to "cool" time travellers like himself, though he laughed off the idea of him being a "Time Detective". Comparing them to his form of time travel with the TARDIS, he playfully laughed off UNIT's attempt to create a Time window and pitied the Rani for having to use a Time Ring, but did marvel at the Time Hotel's operations. He knew to be careful when it came to meddling with time, even forbidding himself and Ruby from visiting her abandonment at the church on Ruby Road due to its status as an important fixed point in time, despite their shared fascinated by the mystery of the event, and knew it could be dangerous to visit himself in the past.

The Fifteenth Doctor did not believe in monsters, instead deciding to see threatening looking beings as just "creatures he hadn't met yet", and would abolish those who used their faith to justify their rash actions in the absence of proof, but admitted his dislike for faith did not mean he would belittle it. He took the betrayal of trust as taboo, and was prepared to disown someone if they broke his trust or sold him out, but was willing to forgive them once he calmed down from the shock of their betrayal. Believing there was "no such thing as a bad way" to save a life, he would encourage scared people to overcome their fears with positive encouragement, but was not afraid to adopt a harsher demeanour to deliberately anger someone if it yielded better results.

He believed uniqueness was a superpower, and thought songs about soldiers "should be sad." He felt that life was defined by death, with "only the best parts" surviving when someone "melted away", and that love persevered "in and out of the slipstreams of time, where beautiful things could be forgotten and gone." While he was normally fearless, the Fifteenth Doctor feared the powers of the Pantheon of Discord, reacting with terror when he realised Maestro was one of "the gods", struggling to hold on to hope of stopping them, even belittling his victory over the Toymaker as an act of pure chance. However, he was able to regain his nerve against Maestro for Ruby's sake and talked down to Sutekh when he gained the upper hand. He also retained a fear for the Midnight entity, being particularly horrified once he realised he was back on the planet Midnight.

The Fifteenth Doctor was a lover of all music, believing that "without music, the human race would go sour." He was particularly a fan of the Beatles, Cilla Black, and Kylie Minogue. He would sometimes speak in a musical tune, and would sing to himself when he needed to keep calm. He liked new things, particularly new ways of looking at the universe, and also loved learning about new locations. He enjoyed skies "ripe with constellations", and loved New Year's Day for the "hopes and possibilities" it inspired. He also had a fondness for unusual names, such as "Lulubelle", "Mundy", and "Joy". He also liked whistles and lions, but disliked having to wait. He also disliked having to follow someone, instead preferring to find out where they were going and trying to get there first. He had a macabre sense of humour when infiltrating the Goblin ship in spite of the open risk of the Goblins eating him, and laughed at the fate of Alan Budd. He was likewise so enthralled by the sight of the Goblin band that he overlooked that Lulubelle was about to be eaten by the Goblin King, deliberately scared the space babies by invoking the name of the Bogeyman, got too focused on coining the word "smelted" to realise Ruby's discomfort with holding a dead body, and momentarily overlooked Belinda's distress at Earth's disappearance when he got distracted by the idea of a giant moth being involved. His favourite places in time and space included the 1960s, Wales, and Omo's Palace, and his favourite species included the Ood. He also held a respect for the Silurians. He retained his eleventh incarnation's enjoyment for fish fingers and custard, and took his coffee with fresh milk and his tea with milk and no sugar.

The Fifteenth Doctor thought himself as being "the best" amongst his other incarnations, and was indignant when his fans favoured a Tenth Doctor adventure over any of his own. In a moment of self-reflection, however, he hesitated to invite Ruby Sunday aboard the TARDIS, thinking he was "bad luck", and later confessing to Kate Lethbridge-Stewart that he saw himself as someone who "bring[s] disaster" wherever he went. He believed that he was a man who was always moving and being positive, even when faced with tragedy, doing his best to avoid seeing people again for fear of bringing harm to them. When Ruby tried to urge him to take some time after the loss of Rogue, the Doctor told her that he was forever moving onwards and upwards to new horizons. However, by the time he met Belinda Chandra on Missbelindachandra One, the Doctor had grown enough to allow himself to grieve for fallen friends. When he was subjected to racial prejudice from the Finetime community, despite having saved them from the mantraps, the Doctor, upon realising that they were racist, having previously not noticed their microaggressions, still asked them to let him save them. When they still rejected him out of fear they would be "contaminated" by him, the Doctor initially laughed off the whole situation, before erupting into a primal scream out of frustration that he wouldn't be able to convince them to come with him in the TARDIS simply due to his skin colour. After finding comfort in the community of Lagos, where he faced no judgment for his skin colour, the Doctor accepted that there were simply places he would not be as welcomed in as he was before, electing to focus on the business at hand when he skirted around segregation laws, opting to "live in" the moment and "shine" as he waited for the people to live in the shrine.

Running towards screams to see if he could help, the Doctor sought to protect life of all sorts, even those with a monstrous appearance or prejudicial attitudes towards him. He prioritised the safety of others even when his own wellbeing was in peril, but was not afraid to risk someone's life for answers if he believed he was in control of the situation, finding himself scared when something was beyond his ability to understand and control. The Fifteenth Doctor acted more on instinct than thought, which could leave him flabbergasted by his own actions. While he had great self-control in keeping his nerve, he would act rashly when driven by his emotions. When someone died following his instructions, he would try to internalise his guilt, but could release his rage in short bursts via striking a wall and shutting down afterwards. When he saw the magnitude Sutekh's Dust of Death was having on the universe after he realised he had been used by Sutekh to place his Angels of Death around time and space, the Doctor was left screaming with rage into the void of a silent universe. While he found it easy to befriend strangers, trying to greet everyone in a room when meeting them and even compliment improvements in their appearances, the Doctor could get too caught up in a mystery to treat someone mysterious as a person. He preferred to sneak off without a farewell, as they made him emotional. He strived to be respectful towards the customs of the lands he visited, and tried to accommodate to a person's preferred method of communication when he could.

Living by the mantra of "moving on", the Doctor knew he "couldn't save everyone" in a crisis, and also chose not to dwell on his own tragedies, lightly telling Ruby Sunday he had no family left before quickly resuming the task at hand. However, when telling her of Gallifrey in the comfort of his TARDIS, the Doctor was more wistful, and could still be triggered when painfully reminded of his destroyed homeworld. He also reflected sombrely on the abandonment and adoption of the Timeless Child when seeing Carla Sunday shower Ruby with affection, and also tried to warn Ruby that her reunion with her biological mother, Louise Miller, could end negatively, specifically pointing out how she never tried to find Ruby and how "unfair" it could be on Louise for Ruby to approach her after using time travel to find her, though he was happy when the reunion was embraced by Louise. No longer wishing to be a "great enigma", as he considered his need to be "mysterious" as the reason behind his loneliness, the Fifteenth Doctor would open up about his past and identity to his newest friends with ease. However, he would bottle up his emotions when it came to tearful goodbyes. He could also be over personal with people he just met, causing him to act without proper consent, though he would apologise when he was called out for his actions, and preferred to speak hard truths than try to falsely alleviate a situation by lying, though would backtrack on his statements to save himself from embarrassment and would indulge in minor deceptions in order to prank his friends.

Not wanting to show off his intelligence, the Doctor had the humility to not think himself a genius, instead prioritizing the love he had experienced in his long life as inspiration for his ideas, though he was still aware of the fact he was an "insane genius".

Traveling through the universe "just for fun", the Fifteenth Doctor enjoyed the feeling of being himself, and embracing the thrill of being alive in memory of fallen friends who could not, preferring to live "in the moment" and "shine". He found the universe to be endlessly entertaining, being amazed by even the smallest of curiosities, and laughing at the weirdness he encountered in his travels. Despite his large personality, he preferred blending in with his surroundings than drawing attention to himself. Enjoying the freedoms that came with his nomadic travels, the Doctor displayed an optimistic and adventurous outlook towards the future, having been freed from the emotional baggage of his past following the Fourteenth Doctor's rehab on Earth, allowing him to be more accepting of his losses, while fondly remembering his past loved ones. Good at deescalating high tensions, the Fifteenth Doctor sought to comfort those he knew were in distress, easily opening himself up as a shoulder to cry on and as an ear for others to air their grievances.

Once the readings from the "Vindicator" helped them make it to 24 May 2025 after they saved the Harmony Arena, the Doctor and Belinda were brought into Conrad's World and forced to play husband and wife with their daughter, Poppy, as part of the Rani's plan to find Omega in the Under-Universe. Once freed from the false reality by Anita and the Time Hotel, the Doctor joined forces with UNIT and his former companions to restore the true reality and prevent the Rani harnessing Omega to bring back the Time Lords in her image. Once the Rani was swallowed by Omega, the Doctor was able to banish him back the Under-Universe and use the God of Wishes, Desiderium, to restore the universe, with Poppy disappearing as a consequence. Unable to accept saving the day at the price of an innocent life, the Doctor sacrificed himself by willingly triggering a regeneration to channel the regeneration energy into the Time Vortex to slightly rewrite reality and restore Poppy.

Following Ruby's departure, the Doctor found himself at the Time Hotel, where he had to stop the Villengard Corporation from harvesting a Star seed, though he had to take a year long break to complete a time-loop, during which he stayed at the Sandringham Hotel and befriended the proprietor, Anita Benn. Once he returned to Time Hotel, he was helped in safely disposing of the Star seed by Joy Almondo, who absorbed the star seed in Bethlehem and took it into space, transforming into the Star of Bethlehem.

After being told he would travel with her while confronting Conrad Clark, the Doctor sought out Belinda Chandra, rescuing her from Missbelindachandra One, but then finding himself unable to pilot the TARDIS to 24 May 2025, resulting in him building a Vortex indicator to help him and Belinda by taking readings in 1952 Miami, 5020th century Midnight, and 2019 Lagos.

After emerging from a bi-generation, the Doctor helped his fourteenth incarnation banish the Toymaker and convinced him to take a rehab with the Noble family to heal from his traumas so that he could move on from the emotional baggage and deteriorating mental state of his predecessors and journey the universe with a newfound optimism, though with the knowledge that the Toymaker's legions were "coming".

The Doctor became acquainted with Ruby Sunday while investigating goblins that were abducting and eating children, and she joined him in his travels once he banished the goblins, though they would be confounded by the mystery involving Ruby's abandonment at Ruby Road on the night of her birth, with the identity of her mother haunting the Doctor in the form of a snowfall that followed Ruby as they saved space babies, clashed with Maestro and their "powers [from] beyond the universe", and survived a landmine on Kastarion 3. They likewise noticed the recurring appearance of a woman in their travels to places such as Wales, finetime, and Pemberton Manor.

The two mysteries would collide with the return of Sutekh, who had hijacked the powers of the TARDIS to deposit incarnations of his "Angel of Death" to every destination the Doctor had visited in order to deliver his Dust of Death to the universe, with his intrigue on Ruby's origin convincing him to strike. After killing Sutekh with the powers of the Time Vortex to reverse his annihilation of life in the universe, the Doctor managed to find Ruby's birth mother and revealed her to be an ordinary woman only given significance by their own obsession with her identity. After helping them make contact, the Doctor left Ruby so she could spend time getting to know her mother and father.

Filled with a lust for life that left him unafraid to show his emotions, the Fifteenth Doctor embodied the ideas of freedom, choosing to journey to the next horizon in his travel with invigorated optimism, purely for the fun of doing so. With the suave attitude to compliment his flamboyant self-confidence, he was more experimental than his preceding incarnations, engaging with people with a newfound perspective from his new skin colour and being more comfortable with romancing people of his own gender. However, he was aware of the backslash he would receive from those that looked down on such things, but ultimately elected to ignore such attitudes when he could and instead "shine" until people overcame their prejudices.

In series 15, the Doctor is joined by a new companion, Belinda Chandra (Varada Sethu).[28] The series follows the Doctor as he attempts to return Belinda to Earth on 24 May 2025. Because the TARDIS cannot land there, he invents a device called the vindicator to create a web of locations across time and space to help him get there. Along the way, the Doctor faxes off with another member of the Pantheon in the form of the god of light, Lux Imperator (Alan Cumming) in "Lux", as well as a former acolyte of various gods of stories in “The Story & the Engine”. On their final stop, in "The Interstellar Song Contest", the pair learn Earth was destroyed on 24 May 2025. In “Wish World”, it is revealed the Doctor’s nemesis the Rani (Archie Panjabi) magnified the powers of the god of wishes, Desiderum, to replace Earth with a fantasy reality that would fracture under its internal contradictions to facilitate the release of the Time Lord founder Omega on 24 May 2025. After Rani and Omega’s defeat in “The Reality War”, the Doctor feels forced to trigger his regeneration in order to subtly alter reality and bring back Poppy, who had been his daughter with Belinda in the Rani’s fantasy world. Thanks to some timely advice from his thirteenth incarnation (Jodie Whittaker), Poppy returns – but now fully human, and no longer the Doctor’s child. Leaving Belinda and Poppy behind, he returns to the TARDIS and completes his regeneration, assuming a familiar face

In contrast to his predecessors, Gatwa portrays a Doctor who is attempting to let go of much of his recent emotional trauma, with a more emotionally balanced, liberated, fun-loving, and affectionate outward persona.

In contrast to previous Doctors, the Fifteenth Doctor's stories include a greater focus on fantasy elements, intended by showrunner Russell T Davies to "increase the danger" and have a "bigger, wider sense of imagination". The Doctor next appears in "The Church on Ruby Road" in which he meets new companion Ruby Sunday and faces off against goblins. In the subsequent episodes, the Doctor and Ruby encounter a surprising amount of magical and supernatural threats, including a fairy circle in "73 Yards" and Maestro, the child of the Toymaker, in "The Devil's Chord". Maestro warns that more evil extra-dimensional gods from their pantheon are pursuing the Doctor, including "The One Who Waits". Ruby herself also exhibits strange qualities when her life is endangered, frequently conjuring the heavy snow from the day she was abandoned at the church of Ruby Road as baby. Late in the season, the Doctor begins a romance with a time and space travelling male bounty hunter known as Rogue in the episode of the same name.

Prior to the first appearance of the Fifteenth Doctor, Gatwa made an uncredited cameo as himself, in costume as the Doctor, during an edited repeat of the 2013 docudrama An Adventure in Space and Time, airing on 23 November 2023, Doctor Who's sixtieth anniversary. In the original version, while preparing to film his regeneration scene, William Hartnell (David Bradley) looks across the TARDIS console and sees a vision of Matt Smith, the actor who played the then-incumbent Eleventh Doctor during the show's fiftieth anniversary year. For the 2023 repeat, the scene was edited to replace Smith with Gatwa.[26]

Gatwa first appears as the Fifteenth Doctor in the 60th anniversary special episode "The Giggle", appearing in the final third of the story as the Fourteenth Doctor (David Tennant) appears to regenerate but instead "bi-generates", introducing the new Doctor as a separate physical entity. Background and Origins: Bi-Generation: The Fifteenth Doctor did not replace the Fourteenth; rather, he split from him in a “bi-generation” event triggered by the Toymaker. Free from Trauma: He is considered to be free of the deep emotional burdens of his past lives, allowing for a more liberated, fun-loving, and "healed" persona.
