Jack eats very sloppily, as a black substance constantly drips onto his organs (this is likely the reason why Edwin's kidney was not eaten in the Mitch story). He can go wild if he is very hungry.

Eyeless Jack is described as a hooded creature with a featureless blue mask with no nose or mouth. A black substance constantly oozes from the eyeholes on the mask. The character's creator reveals that beneath the mask, Jack has no eyes, gray skin, shark-like teeth, and brown hair. The original story's protagonist, Mitch, notes that Jack has claws.

According to the original story, a boy named Mitch moves in with his brother, Edwin. After a week of living there, Mitch wakes up with a large wound on his cheek. At the hospital, he's told he's also lost his left kidney, which was surgically removed during the night. Around midnight, Mitch wakes up to see Eyeless Jack leaning over his bed. Mitch quickly grabs his camera and takes a photo before Jack attacks him. Mitch fights him off and runs into the woods, where he trips and falls unconscious. When Mitch awakens in the hospital, he's told that Edwin has been killed. Mitch and Edwin's parents take Mitch home to collect his things. When Mitch enters the house, he sees Edwin's corpse and something next to it. Later, upon examining it more closely, Mitch realizes it's his own left kidney, gnawed and covered in some kind of black substance.

As for the character's history, he doesn't have a backstory as such, but there is a brief factoid stating that Jack was once an ordinary man, but under unknown circumstances was used as a sacrifice in a human sacrifice ritual. During the ritual, his eyes were gouged out and filled with resin. Ultimately, the ritual failed, and Jack escaped, becoming a monster with no memory of his past life and a human-eating nature.
