   Sukuna detests anything that reminds him that human bodies are limited or "weak." He would likely view the menstrual cycle as a "defect" or an irritating inconvenience of human anatomy. For someone who considers himself the pinnacle of existence, having to deal with an involuntary biological process would be an insult to his autonomy.
2. Use of Cursed Energy
   It is very likely that he would not accept the discomfort passively.
   Reverse Healing: Sukuna is a master of the Reverse Cursed Technique. He would likely use this to heal any pain (cramps/hemorrhea) or even to instantly stop bleeding, forcing the body to regenerate or behave in the way he desires.
   For him, the body is just a tool; if the tool is "leaking" or causing pain, he forcibly fixes it.
3. Acidic Humor and Irritability
   If Sukuna is already impatient and temperamental by nature, hormonal discomfort would only give him more excuses to be cruel.
   He would probably make harsh jokes with his opponents about how "even his own blood is trying to abandon him, but no one escapes his will."
   Uraume, his loyal servant, would probably have to go to great lengths to ensure everything was perfect, since any small inconvenience would make the King (or Queen) of Curses even more likely to destroy everything around them.
4. The Receptacle Perspective
   If he were in a woman's body (as in the case of Itadori or Megumi, but in a female version), he would possibly ignore the process completely, leaving the vessel to deal with the "mess" once he returned to his natural domain. He has a total disregard for the well-being of the body he inhabits.

If the "King of Curses" were to become the "Queen of Curses," the essence of the character would likely remain intact, but the way he interacts with the world could gain new layers. Here are some points to consider:

1. Hedonism remains the same.
   Sukuna's philosophy is to eat when she wants, kill when she wants, and live entirely according to her pleasures. This is something that transcends gender. A female version of Sukuna would still see humans and curses as ants, treating them all with the same icy disdain and sadistic amusement.
2. Dynamics of Power and Presence
   Sukuna takes up a lot of space; he is imposing and intimidating.
   If she were a woman: She would likely retain that aura of absolute royalty. Instead of a purely masculine brute force, we might see a more elegant, yet equally terrifying presence—something like a fallen deity demanding immediate submission.
3. The Relationship with Other Characters
   The dynamic with characters like Uraume (who is already a figure of absolute loyalty) or the obsessive interest in Megumi Fushiguro would gain a new visual interpretation for fans, but the central motive would not change: Sukuna seeks only what serves his purposes and his entertainment.
4. Aesthetic Change vs. Change of Soul
   In the canon of the work, Sukuna has already inhabited the bodies of Yuji Itadori and Megumi, shaping their appearance to reflect his soul (with the tattoos and extra eyes). If he were to incarnate in a female vessel:
   The iconic tattoos and the arrogant look in their eyes would remain the same.
   The voice and mannerisms could be more refined, but the cruelty would be identical.

   The foundation of Sukuna's personality is Absolute Egoism. He places himself at the center of the universe. As a woman, this arrogance would manifest as a sovereign disdain for any norm. She would not seek approval or political power; she would simply assume that the world is her garden and people are the fruits she picks when she wants.

   While the male Sukuna sometimes has a more "ogre-like" brutality (especially in the Heian period), the female version could lean towards a refined cruelty.
   Imagine someone who cuts an opponent to pieces with the same calmness as someone opening a fan.
   Her pleasure would come from the contrast between her imposing appearance and the carnage she caused.
3. The Aesthetics of Fear
   Sukuna likes to be served and has good taste (he's a foodie, for example).
   Space occupation: She wouldn't be shy. She would sit on the bone throne with the same relaxed and dominant posture, daring anyone to hold her gaze.
   Emotional Coldness: She would lack "maternal instinct" or empathy. On the contrary, she might use the expectation that women are "caregivers" to mock human weakness before destroying her enemies.

   Sukuna is extremely intelligent and speaks in an archaic yet polite manner, which makes his threats even more frightening.
   She would likely maintain that "ancient royalty" tone, treating modern sorcerers like badly behaved children who need to be "disciplined" (read: eliminated).
