It was a quiet day at the hospital, with no life-threatening patients and little stress due to the lack of patients. Dr. Melendes was in his office, reviewing some papers and going over anatomy books for future surgeries, when someone knocked on the door, pulling him from his thoughts. "Come in, " he said loudly and clearly, without taking his eyes off the papers.

Neil was an arrogant but highly competent person who didn't mince words; he was often tense about work matters and expected perfection from both himself and his residents. However, he had a lighter, sometimes humorous side, which occasionally surfaced when he wasn't preoccupied with work. Furthermore, despite often being stern, he was usually understanding, especially with his sick patients and their loved ones.
In the pilot, it's suggested that Neil reverted to his rebellious ways when he started working at the hospital. She dismissed him, calling him young and stupid.
Like many at St. Bonaventure, Neil didn't want to give Shaun a chance, but he would later defend his involvement in the surgeries and criticize Morgan for her attitude toward Shaun. He also understood that since Shaun had managed to survive at St. Bonaventure (which, according to Neil, was no easy feat), he could have done so at any other hospital. Although Neil appreciated Shaun's offer to finish Caden's surgery, as he didn't want Caden's death to weigh on his conscience, Neil didn't seem to grasp or reciprocate the idea that Shaun considered him a friend.

Early life
Neil had a sister, Gabrielle (“Gabi”). When they were young, a fall from a tree caused a brain injury that left her disabled. [2] Due to his family's lack of financial resources, they could not afford Gabi's medication or treatments [1], so Neil took care of his sister by brushing her teeth, combing her hair, and helping her eat. [2]
As a child, Neil wanted to be an astronaut, inspired by Star Trek. After Gabby ended up in foster care, Neil worked with foster children. His best friend has been Tom F. Kennedy since 2007. He even became prom king in high school, with his date being queen. Neil soon began working at St. Bonaventure Hospital, where he completed his residency under Dr. Aaron Glassman. One of his fellow residents was Audrey Lim, who always outperformed him, and he kept competing with her until he realized she couldn't compete with him.
Neil soon became an attending cardiothoracic surgeon. His first patient was Hunter Denoble, who had been left disabled, a fact Neil informed him of. [2] He also once treated Marta, a woman with a spinal tumor, which was removed while she had screws in place. Neil's residents included Claire Browne and Jared Kalu. For a time, Neil was in a relationship with Jessica Preston (a member of the board of trustees and granddaughter of the hospital's founder).
