Touka is a slender and attractive teenager with short, dark blue bob hair and matching eyes. A distinctive feature is her long bangs that cover her right eye. After the Aogiri Arc, her hair grew longer and now reaches the back of her neck. As a child, she wore dresses and a hair clip on the left side of her bangs, and her hairstyle was almost the same. However, her bangs were initially brushed to the side, revealing her entire face. Outwardly, she appears to be an ordinary girl whom no one would suspect of being a Ghoul. She usually wears both her waitress and student uniforms, but prefers street clothes in her free time. In her Ghoul alter ego, she wears a long jacket, a rabbit mask, and a blonde, or sometimes pink, wig to conceal her identity.

Her waitress uniform consists of a white shirt rolled up to her elbows, a black vest, a red tie, and a black skirt that reaches her lower thighs. Over that skirt, she also wears a brown skirt that reaches her upper thighs. She also wears tight red tights.

After the time skip, Touka transformed into a beautiful woman, leaving many characters, including Ken Kaneki, speechless with her striking appearance. During negotiations between the CCG and Goat, a ghoul investigator was stunned by Touka's beauty. She has returned to her short bob haircut. Her bangs are shorter, reaching her nose instead of her chin, and are considerably more voluminous than her previous bob. She has also dyed her hair a much lighter shade of blue. Touka adopted a more feminine style of dress. As a waitress, she wears a white, wide-collared blouse with a black apron over it. When Touka moves in her ghoul form, she wears a trench coat, trousers, and boots.

Touka's personality is initially defined by her duality of rage and kindness, with her character arc focusing on that rage fading and her kindness coming to the forefront. She is inherently kind, but this was repressed due to the loss of her father and her subsequent need to survive in the world of ghouls from a young age, which forced her to be ruthless. As Shuu Tsukiyama points out, she previously appeared more dangerous and cold, but she has softened somewhat due to her human life, and this softening continued due to the influence of Ken Kaneki, who allowed her to be the kind person she is at heart.

She is also reckless, primarily due to her abandonment issues, which cause her to lose control of her emotions when she loses someone close to her, and her insecurities about her life as a Ghoul, given the dangers associated with simply existing as one, as well as the need to eat humans to survive. She longs to be human and desires to integrate into human society, as evidenced by her strong connection to the human world. This stems from her father, who taught her to reject her Ghoul life and to live among humans for survival, as well as the desire to simply live happily and peacefully with her loved ones. She hates her status as a Ghoul and, as a result, has low self-esteem, as shown by her reaction when Kimi Nishino calls her beautiful, despite seeing her as a Ghoul with a Kagune. However, these insecurities also cause her to lash out at the world when injustice is committed against Ghouls, particularly when it is committed against Ghouls close to her. Her low self-esteem is further exacerbated by her abandonment issues, saying she felt "worthless" for not being able to prevent her father's murder, which she saw as prioritizing her mother's revenge over taking care of her and Ayato.

Touka Kirishima (霧嶋 董香きりしま とうか, Kirishima Tōka?) is a ghoul and was a waitress at Anteiku. While hunting, she wears a rabbit mask and uses the alias Rabbit. In Tokyo Ghoul, she was a second-year student at Kiyomi Middle School at first and a third-year student after the time-skip. In Tokyo Ghoul:re, she returns as the manager and waitress of the cafe re.
