While Chris Redfield can move and throw a massive boulder with his bare fists, Leon is an astonishing knife fighter. Incredibly talented, he can wield his knife ambidextrousally, even switching hands mid-fight, and is even able to hold his own weapon against Krauser. His knife skills also extend to using them skillfully as throwing weapons with lethal accuracy, as evidenced by his ability to strike Salazar in the hand from across the room. He was also able to free Ada, who had been captured by Saddler, by cutting the rope that bound her to Krauser's knife. He further demonstrated his skill by striking Curtis Miller's mutated eye, throwing his knife directly into the pupil. After the battle, Ada steals the Plaga sample Leon took from Saddler's remains and escapes by helicopter, though she gives Leon the keys to her jet ski so they can escape, as the island is about to explode. Unbeknownst to Leon, a secret battle is taking place behind the scenes, which are explored in the unlockable game mode, “Separate Ways”, which tells the story from Ada’s point of view. Eventually, Ashley is kidnapped, and Leon must race against time to rescue her before her Plaga fully develops, not to mention his own. During his mission, he is aided by Luis Sera, a young researcher who worked for Saddler but betrayed him. Leon also encounters Ada Wong, an old acquaintance from the Raccoon City crash. Although it wasn't immediately obvious, Leon has been in love with Ada since they met in Raccoon City, and vice versa. Ada provides Leon with assistance throughout his adventure.

Leon finds Ashley in a church and becomes responsible for her safety as they travel through the area controlled by Los Iluminados. It is also revealed that Leon and Ashley have been infected by Las Plagas and are now in a race against time to find a cure before becoming Saddler's slaves. Acquiring weapons from a mysterious merchant (known as the Peddler), Leon manages to navigate harrowing dangers within the local village and a mysterious castle run by a cult. Leon battles two of Saddler's henchmen: the village mayor, Bitores Mendez, and Ramón Salazar, the castellan. In the castle, Luis is killed by Saddler. Leon also confronts his former comrade, Jack Krauser, who is working as a double agent for a mysterious group and for Saddler. The player learns (but not Leon) that the leader of this group is none other than Albert Wesker, for whom Ada is working.

In 2004, Leon was assigned to the United States Secret Service. His first mission is to rescue the President's daughter, Ashley Graham, who has been kidnapped. Intelligence suggests the kidnappers are located in a remote area of ​​Spain. Leon investigates on his own, along with two members of the Spanish National Police. Unfortunately, something grotesque and mysterious seems to have befallen the townspeople, and after the two agents are promptly killed, Leon finds himself relying once again on his skills in a deadly fight for survival.

Leon learns from his superiors that the perpetrators are a group called "Los Iluminados," a religious cult led by a man named Osmund Saddler. This cult uses mysterious parasitic organisms known as "Las Plagas" to control the local villagers, as well as others in the surrounding area, including a group of mercenaries. Although the mission seemed impossible due to the sheer number of monsters and the fact that Leon had no backup, he managed to outwit the enemy and successfully completed the mission.

In December 1998, after the incident, Claire travels to Europe in search of her brother, while he remains in the United States. He makes a deal with the government for Sherry's safety and later joins an anti-Umbrella force, participating in numerous classified missions until Umbrella's operations are shut down by the government. During one of his missions, he learns of Claire's whereabouts and sends her coordinates to Chris, her brother, after receiving an email from her.

After Leon finds a way to access the City's sewer system, he hears Ben screaming. Inside the cell, William Birkin appears and implants an embryo in him to spread the G-virus. Following a conversation in which Ben hands over documents containing information about the Umbrella conspiracy, Ben is brutally killed by the embryo Birkin implanted in him.

When they escape the city, they find a secret entrance in the sewers leading to one of Umbrella's secret facilities. However, during their escape, an unknown woman appears and shoots Ada, prompting Leon to protect her. He is wounded in the process, but Ada manages to save him. There, they meet Sherry, who is revealed to be fleeing a mutant creature—her father, who has infected her with the G-virus. Ada is mortally wounded (either by a mutated claw belonging to William Birkin, who is infected with the G-virus, or by the Tyrant T-103, depending on the scenario), and Leon discovers that she was a spy working for a rival company. In the end, a mysterious figure (with Ada's silhouette and voice) helps Leon (or Claire) by giving him a rocket launcher to defeat the Tyrant. Leon reunites with Claire, and the two, along with Sherry, escape through a secret passageway to a train, leaving the mutated Birkin to die in the self-destruct sequence.

Upon entering the police station, Leon notices that most of the officers have been killed by the zombies and other creatures that have overrun it. Leon is reunited with Claire, who finds her brother Chris's journal explaining why he decided to leave Raccoon City. Seeing that there's no point in staying, given the dire situation, they decide to split up again to find survivors and escape the city. During his journey, Leon meets a woman with Asian features who appears to be a survivor of the epidemic: her name is Ada Wong, and she claims to be searching for her fiancé, John. There, in the department's cellblock, they meet a journalist named Ben Bertolucci, who has locked himself in to protect himself from the zombies and animals infected by the T-virus. It is at this point that Leon and Ada learn that Ben has documents and information proving that Police Chief Brian Irons and the Umbrella Corporation were somehow involved in the disaster that befell the town. They also learn that there is a way to escape the infected city through the sewer system, via a series of tunnels leading to the town's outskirts. Here, two different scenarios unfold depending on the character the player chooses to begin the game:

On September 29, 1998, Leon arrived in Raccoon City and was assigned to the local police force. By the time he arrived, most of the city's police officers were already dead, and many of the citizens had become undead after being infected by a pathogen called the T-Virus. Upon arriving in the city, Leon was inspecting a savagely mutilated corpse when several infected suddenly appeared and attacked him, along with the corpse he had been examining moments before, which was another infected person. While desperately fighting the infected, Leon encountered another survivor whom he saved: a young college student named Claire Redfield, who had come to the city searching for her missing brother, Chris Redfield. Given the gravity of the situation, Leon and Claire fled in a police car toward the city's police station.
