Johan Liebert is a serial killer and former patient of Dr. Kenzo Tenma, as well as the older twin brother of Nina Fortner, formerly known as Anna Liebert. When Johan and his sister were children, Johan was shot in the head during an alleged home invasion. Tenma saved his life (albeit at the cost of his career), leaving Johan deeply indebted to him.

Appearance Johan has very light blond, short hair, parted in the middle, light blue eyes, and pale skin. He is often described as exceptionally handsome and beautiful. His slender build and delicate facial features allow him to easily pass for a woman, and he takes advantage of this on several occasions.

As a child, he had short bangs that fell over his forehead.

Personality On the surface, Johan is a polite, charismatic, and compassionate young man, albeit somewhat distant, who possesses a myriad of admirable qualities. This meticulously constructed facade facilitates the acquisition of new allies and their manipulation into performing various tasks for him, through the influence of their supposed friendship and goodwill. With such skill in creating this aura of perfection and purity, Johan easily conceals his true motives.

Behind this artificial facade, however, Johan sows destruction and suffering to those who fall into his traps. Much is revealed about his true nature, since even before his time at Kinderheim 511 and the horrific experiences he endured there, Johan already demonstrated psychopathic tendencies and dark personality traits. Most of his murders, both in childhood and adulthood, were premeditated and calculated. Many of Johan's beliefs, judging by his actions and words, seem to align with nihilism, as he sees no meaning in life. He states: "Most of this universe is 'just death' anyway," and that for the universe, "most lives are just grains of sand in a corner of the Earth, which disappear in an instant."

Johan also has a tendency to make his victims experience the worst possible loss and suffering, rather than killing them himself; this experience, however, often leads the victims to suicide.

The origin of Johan's actions is unknown. Neither of their parents appeared to suffer from psychopathy or any other mental illness. Nina, despite being the twin who experienced the traumatic events at the Red Rose Mansion, reveals herself to be a genuinely loving and caring person. However, while she manages to suppress her memories of the Red Rose Mansion, Johan apparently assumes them as his own, even believing that he was the one who experienced them.
