Annihilation John Doe has a mostly black body, with a black head, torso, and legs; his arms are reddish-pink. His arms are segmented into two blocky parts: one with floating, triangular fingers and the other with a sharp spike protruding from it, surrounded by black spikes. In-game, his torso and legs have a reddish-pink grid pattern. The original corruption on his feet is now square and reddish-pink. One of his eyes has reddish-pink blocky corruption particles emanating from it, and the eye itself is completely black with a reddish-pink outline; his other eye is completely white with a reddish-pink outline. John Doe of Annihilation wears a pink and red crown with a shape similar to Pwnage's Black Iron Crown at Milestone IV, a cape with a gradient from pink and red to black, and a pink and red collar with a white triangle with a thick pink and red outline.
