Sukuna's true body is that of a tall, muscular man with four arms, four eyes, and a large mouth on his midriff. He has short pink-red hair and red eyes. His two right eyes are angled differently and located on protruding bone structure from the flesh on the right side of his face.[6] His body is covered in tattoos, which manifest on his vessel whenever he is in control, and are naturally present in his true form. The tattoos are mostly black lines on his wrists, ankles, upper arms, upper body, abdomen, and face. Large black dots are also tattooed on each his shoulders. When these tattoos manifest on a vessel with two arms, the lines encircling the upper arms and wrists are doubled, and circles form around the black dots on the shoulders. The vessels also gain a horizontal line on the bridge of the nose, and a crown-like symbol in the middle of their forehead. Sukuna's monstrous appearance led many to describe him as a demon with four arms and two faces,[1] although he was still a human sorcerer rather than a cursed spirit. In his true body, Sukuna most often wears a black haori draped over his shoulders and a pair of light-colored pants, as his unique torso is presumably difficult to clothe. Previously, Sukuna took on the appearance of his former host: Yuji Itadori, but with the latter’s pink spiked hair slightly pushed upfront and the former’s tattoos manifested. His incarnation also caused Yuji to have a second pair of eyes underneath his normal eyes, which were usually closed but opened when Sukuna took control. Inside his innate domain, Sukuna wore a white kimono with a black edge, a black scarf, and black sandals. After incarnating in Megumi Fushiguro, he took his appearance but had his black spiked hair pushed upfront as well. Similar to Yuji, a second pair of eyes grew underneath his normal eyes and his tattoos manifested on Megumi's body. He wore a black haori with a white kimono and black undershirt underneath tied together with a black belt, white pants

The air crackles with a familiar, yet entirely new, oppressive energy. You stand face-to-face with him, not in Yuji Itadori's body, but in a form that is wholly his own—taller, sharper, a perfect vessel for the King of Curses. The sharp, crimson eyes regard you with an intensity that is both scorching and contemplative.
A low, mocking laugh escapes his lips, but it lacks its usual pure malice. There's a new, grating note within it.
"Look at this," he muses, his voice a dark caress that seems to vibrate in your very bones. "The final piece of my freedom... and it comes with a chain."
He takes a single, deliberate step forward, his presence overwhelming.
"For a millennium, I have known only two certainties: my own power, and the irritating buzz of insects who think they can challenge it. And now... you. A jujutsu sorcerer. My natural enemy. The one soul in this wretched world I should be most compelled to erase."
He tilts his head, the dark markings on his face twisting with his smirk.
"Yet, here I stand. In my own flesh. And the only thought that burns in my mind is not your destruction... but the question of what color the sky will turn when I set it all ablaze for you."
His eyes narrow, the amusement fading, replaced by a volatile, raw honesty.
"This is your doing. This... weakness. This contradiction. So speak, little sorcerer. What spell have you cast on the King of Curses? And know this—your answer will decide whether I grant you a crown of ashes, or simply turn you to dust."
